Vision Service Plan (VSP) www.vsp.com
VSP offers two vision coverage levels: Basic and Premier. The basic option is free and automatically enrolled with your health plan; the premier level is provided at an additional monthly fee and is voluntary. Only employees and dependents enrolled in a health plan are eligible for coverage in both the basic and premier plans.

Delta Dental is a PPO dental plan that allows the flexibility of seeing in-network and out-of-network providers while offering up to $1500 of coverage, annually. Here’s how it works:

Extended Sick Leave | After all earned and accumulated sick leave is exhausted, 100 extended sick days will be available each year for use while sick or injured.

Transferring Sick Leave from your previous school district to SFUSD | Employees joining SFUSD from another California Public School District may transfer unused sick leave from their previous district, provided they complete the “Transfer of Unused Sick Leave form.” New hires: a link to this form is included in the Chalk Schools Benefits Packet. |
Life Insurance AD&D and Long-Term Disability (Symetra):

What is the cost? | This benefit is provided free of charge. |
How do I enroll? | Employees are automatically enrolled upon hire; no paperwork is required. Coverage begins effective the first of the month following your start date. |
How do I assign a beneficiary? | For existing employees, you may enter your beneficiary elections into EMPowerSF during Open Enrollment. New Hires: You can enroll beneficiaries onto your life insurance policy via EMPowerSF. Details are included in the EmpowerSF Self Service Guide. |
How does my family submit a claim? | Life insurance, AD&D and Long-term disability claims must be submitted by a SFUSD benefits analyst directly. Please see “Key Contacts” for Benefits Analyst availability. |
SFUSD educators participate in the California State Teachers’ Retirement System (CalSTRS), a defined benefit pension plan. | |
What is it? | The CalSTRS Pension plan provides service retirement benefits calculated using a formula based on age at retirement, years of service and final compensation. |
Do I have to be a member? | Yes, CalSTRS membership is mandatory for employees working .5 FTE or greater and begins the first day of employment. |
How much do I contribute? |

Does the district match my contribution? | Yes, SFUSD will match your contribution equal to 19.10% of your base wage. |
What is the vesting period? | You are vested after five years. If you end employment with SFUSD before vesting, you can request a refund of your contribution amount from CalSTRS or roll it into a tax-sheltered annuity. |
What about Social Security (SSA) and State Disability (SDI)? | As a CalSTRS member, you will no longer contribute to Social Security and State Disability. Deductions of 1.45% are still taken for Medicare. |

What is it? | A pre-tax benefit account used to pay for eligible medical, dental and vision care expenses that aren’t covered by your insurance plan. | A pre-tax benefit account used to pay for dependent care services for dependents up to age 13. |

I currently have an FSA and want it again next year, what do I do? | Existing account holders requesting to re-elect their FSA are required to enroll each October during Open Enrollment. You can enroll by logging into your WageWorks account at www.wageworks.com. | |

Am I Eligible? | Certificated working at least .5 FTE (or more) may enroll themselves and any eligible dependents in one of the four health plans provided by SFUSD. This includes tenured, temporary, emergency and intern teachers. |

Health Benefits FAQ (Frequently Asked Questions) continued: | |
I did not receive an ID card for Vision or Dental, what do I do? | Both VSP and Delta Dental do not issue ID cards; your SSN is your ID number (dependent SSN’s are theirs). Once enrolled, you can visit www.vsp.com and www.deltadentalins.com to register an account, check coverage, locate providers and view claims. |
Will SFUSD reimburse me if I do not elect benefits? | SFUSD does not offer reimbursement of health premiums to employees who waive coverage. |
How do I pay my health premium? | Health insurance premiums are deducted from your monthly paycheck. The deduction is taken pre-tax, reducing your monthly taxable income. |
What is HSS (Health Service System)? | Your health insurance plan is administered by Health Services System (HSS) of the City and County of San Francisco. You can call or visit their office with questions about your health plan and vision coverage, Mon - Fri from 8AM - 5PM. HSS contact information is located in the “Key Contacts” section of this presentation. |
What if I leave SFUSD? | Health, vision and dental coverage will terminate on the first day of the month following your last day of work. You will receive a notice to extend benefits under COBRA within 45 business days of your resignation/termination. |
What about Open Enrollment? | Open Enrollment is a period of time during which employees can enroll in benefits, make plan changes or terminate coverage without need of a qualifying event. The annual Open Enrollment period takes place during the entire month of October and any changes will become effective on January 1st of the following year. |
