Documentary Requirements

  • In the absence of PSA Birth Certificate, the parent or guardian must submit a Birth Certificate from the local civil registrar, Baptismal or a Barangay Certification containing the basic information of the child if the learner is not yet registered.

  • Reiterating DO. 58, S. 2017, the birth certificate shall be an attachment to the learner’s permanent record

  • New records, Balik-Aral or with enrolment gap in LIS history are subject for Division Planning Officer’s review and action

Transfer of Credential

Scanned/Image Copy of Form 137 for outgoing Grade 6 and SF10 for outgoing Grade 10 together with the attachments can now be an option/alternative to hard copy provided that:

  • Grades by learning area/quarter are completed
  • Dully signed by the adviser and school head
  • Use of official school deped email schoolid@deped.gov.ph for both schools

*Kinder completer who opted to transfer to another school may also send scanned/image copy of certificate/other credential

For SY 2021-2022, deadlines for the submission of documentary requirements required under Section V-A of DepEd Order 3, s. 2018 shall be deferred to December 2021. This shall apply in both public and private schools.

  • In this time of pandemic, the LIS record of migration of learners from private schools to public schools is one of the indicators being monitored to provide support/assistance for both sectors of education.

  • The LIS Enrollment History record is one of the measure to validate authenticity of the learner’s enrollment claim.

  • In time of calamity/disaster such as the Yolanda Storm Surge, Marawi Siege and Taal Volcano Eruption, the LIS Enrollment Data was used to facilitate and validate enrollment of affected learners who lost/destroyed paper-based record of enrollment credential.

How The Tracking System Works

The tracking system is highly dependent on the “cooperation” of both originating and receiving schools or CLCs.

a. The receiving school (School 2) initiates the request upon enrollment of learner in LIS. The system will notify the originating school and validate if the proposed transfer is legitimate.

b. The originating school (school 1) is expected to respond within thirty (30) days upon received of the electronic notification. The receiving school must decide either confirm or decline the requested transfer.

CONFIRMING TRANSFER REQUEST

By confirming legitimacy of a requested transfer, the originating school indicates the date of last attendance of learner in their school and the date when the credential document sent thru courier or in any form as possible and safe except hand it over to the parent or party with interest. There is also an information box that the originating school can use to provide additional information as needed.

Challenges of Tracking System

  • Some schools take some time in enrolling a transferred learner in the system. They want first to observe the attendance performance so that if the learner stops attending classes, it will not be counted as dropped out in their school. As a result, the originating school has no idea or information about the current school of their former learner. Enrollment is being done at the end of School Year causing multiple activities in the system during EoSY updating

  • Some private schools intentionally delay confirmation of transfer to force parent to settle unpaid financial obligation.

  • Some schools do not check regularly the transfer requests pending in their account.
