Youth Soccer Camp

When: Sat. July 27th

Time: 9:00AM-12:00PM. Registration to start at 8:30AM.

Where: New Haven Soccer Fields (Behind NH School)

Address: 489 High St. New Haven, KY

Who: Boys OR Girls ages 4-13

Cost: $10. All proceeds will benefit SNYSA facility.

Current Lady Generals, TNHS coaching staff, SNYSA, and volunteers will work with youth players on skill development and competitive games. Free pizza and drinks for participants will be provided at the conclusion of the camp.

Medical Release Form for Camp

We the undersigned approve of our child’s participation in the 2019 Youth Soccer Camp hosted by TNHS Lady Generals & South Nelson Youth Soccer Association (SNYSA). We verify that they are in good health and are able to participate in all camp activities and drills. If medical attention is required for illness or injury, we the undersigned hereby appoint, the Thomas Nelson & SNYSA directors, to authorize medical treatment for any injury or illness that may develop during the soccer camp. We the undersigned, hereby specifically waive, give up, and release all Thomas Nelson & SNYSA camp staff members, and Thomas Nelson High School and The New Haven School from any and all claims and liabilities, present and future resulting in any injury or illness which may be sustained or contracted while attending 2019 Youth Soccer Camp.
