What is an All-Payer Claims Database?

  • All-payer claims databases (APCDs) systematically collect administrative data, including medical, pharmacy, and dental claims, eligibility files, and provider (physician and facility) files.
  • These claims are created when an insured patient receives care or fills a prescription, and include a record of what was provided, who provided it, how much was charged, and how much was paid.

What information will be included?

  • Commercial health insurers (fully insured)

  • How much do Alaskans spend on healthcare services?
  • Facilitating price transparency and consumer price shopping for care
  • Evaluating state health reforms
  • What are the trends in healthcare spending for specific chronic health conditions and risk factors?
  • Supporting health system change such as implementation of accountable care organizations or other alternative payment models

How will the APCD be funded?

  • APCDs in other states generally utilize multiple sources of funding to establish and operate APCDs.
  • This includes general fund appropriations, philanthropic and other grants, federal financial participation, and data use/licensing fees.
  • Federal funding of $2.5M for each state was included as part of the recent Consolidated Appropriations Act for APCD start-up or expansion.

Isn’t this data already collected?

  • Yes and no. There are currently a number of data sets that exist that have been or can be used to characterize the Alaska health care marketplace.
    • they are significantly lagged
    • the data is aggregated to a degree that limits its usefulness for policy analysis
    • the data is compiled by the federal government and therefore is outside the control of Alaska

How is privacy and security of healthcare data maintained?

  • The Alaska APCD would collect de-identified enrollment files and provider files that include cost and quality metrics from private and public payers, with data from all settings of care that permit the systematic analysis of health care delivery.
  • The information contained in the database are confidential and not subject to public records inspection or disclosure
  • Individually identifiable health care information contained in the database may be accessed only by the organization selected to coordinate and manage the all-payer health claims database.
