�Community Nurse Connection will be recognized as a national leader in promoting, developing, and supporting community nursing programs
�Goal 1: Create a communication plan that includes key messages, key audiences, and measures of success using a strategic calendar.
Timeline: To begin within one to three months after board approval of the Strategic Plan.

Community Nurse Connection will ensure financial security and ethical and effective governance.
Goal 1: Create a sound fiscal management plan that will include methods to secure stable and diverse sources of base revenue and a scheduled fundraising plan to meet projected expenses.
Goal 2: Establish an effective infrastructure that includes clear roles and responsibilities for the board, staff and committees.
Goal 3: Advance Community Nursing with cultural humility in order to promote diversity, equity and inclusion (DEI).

Community Nurse Connection will support programs and their nurses in providing superb care to their communities and partner with them to adapt and innovate to the changing needs of healthcare
Goal 1: Support the development of new community nursing programs in NH and VT
Goal 2: Create outreach opportunities to share the model beyond our immediate region to promote community nursing
Goal 3: Maintain clinically excellent and compassionate support to all communities in the Community Nurse Connection network.

Advocacy Pillar
Community Nursing Connection will be recognized as a leader on healthcare policy to advance and promote the community nursing model.
Goal 1: Establish a new healthcare policy committee comprised of nurses, board member and representatives from community nursing programs from within the Community Nursing Connection network to address policy and legislative priorities
Goal 2: Create a wide range of advocacy opportunities through presentations, social media campaigns, sharing data to show evidence of the benefits of the Community Nursing model.
Timeline: Begin by 5.2022
