How do you open a new "form" in Google?
Steps:
1). Go to docs.google.com
2). Click on and then click on "form"
This is what your form will look like at first:
It is time to create your own questions...
Let's get personal...
give your form a title and some directions!

Introduction
Now create some questions...
Sample of a
Completed Question
1. Write in your question under
"Question Title"
2. Pick a "Question Type"
3. Create answer choices to
any questions that is necessary
4. If you want to require those
responding to answer that
question, check the box
5. Click DONE
To add more questions,
click "Add Item"
What if you make a mistake after you submit your question???
Use the
edit
tool!

This tool allows you to get back into the question you created and make changes!
Now it is time to put on the finishing touches...
Pick a background theme:

Make a choice of what will fit best with your form or leave it plain...
Save Your Work...and your form is complete!
