How do you open a new "form" in Google?

Steps:

1).  Go to docs.google.com

2).  Click on                         and then click on "form"

It is time to create your own questions...

Let's get personal...

give your form a title and some directions!

Now create some questions...

1. Write in your question under

     "Question Title"

2.  Pick a "Question Type"

3.  Create answer choices to 

     any questions that is necessary 

4. If you want to require those 

     responding to answer that 

      question, check the box

5. Click DONE  

To add more questions, 

click "Add Item"
