Our final strategy

  • Goal: To increase participation and retention in local evidence- based lifestyle change programs for people with prediabetes or diabetes, especially in underserved groups
    • Assess the prediabetes/diabetes landscape
    • Assess current diabetes prevention and management programs
    • Identify barriers to enrollment & retention
    • Increase number of providers (DPP, DSMT, and DSMP)
    • Develop recruitment & implementation strategies
    • Expand local DPP programming; increase DSMP and DSMT services

How we got to the strategy

  • LHIC Chronic Health Work Group:
    • Two active Subcommittees: Youth Obesity Prevention & Colorectal Cancer Screening
    • NEW: Diabetes Subcommittee
  • Dec. 2020: Western Regional Diabetes Partnership Catalyst Grant awarded
    • Frederick Diabetes Partnership (FDP)
  • Jan. 2020: FCHCC Board voted to align their local diabetes initiative with the goals of the Regional Diabetes grant
    • FCHCC Board designated the FDP as the Diabetes Subcommittee

  • Facilitators:
    • Coordinated leadership of the Diabetes Subcommittee and the Frederick Diabetes Partnership (FDP)
    • LHIC Coordinator
      • Bridge between Coalition Board and FDP activities
      • Pilot DSMP virtual workshops
    • New Diabetes Patient Referral Navigator

  • Barriers:
    • Too few evidence-based diabetes lifestyle programs and coaches
    • Targeted neighborhoods with known negative SDOH characteristics
    • Sustainability of increased programming

  • LHIC Engagement:
    • May 2021: Diabetes initiative presented to the community -- Open Coalition Board meeting
    • Broad Community Representation on the Planning Committee
      • Healthcare, Private/Public partners, Community members
    • June 2021: Diabetes Planning Summit
    • Representation of diabetes & lifestyle programming beyond DPP

  • Feedback:
    • Barriers to participation: Transportation, childcare, language
    • Sustainability: Billing for DPP, DSMT & Umbrella Billing
