Obtain the required documents prior to beginning your online application.

  • Obtain transcripts documenting your degree
    • IF you hold an associate's degree, you must ALSO submit evidence that you have successfully completed the district-selected substitute training. IF you hold a bachelor's degree or higher, this documentation is NOT required.  Acceptable documentation of successful completion can be shown through:
      • CESA Certificate of Completion, including date of completion

  • Scan the documents into a file(s) that is 2 MB or smaller.  Acceptable files are:
    • .pdf
    • .doc/docx
    • .jpg/jpeg
    • .txt

The agency number is a district’s 4-digit LEA number with four 0’s added onto the end, to make an 8-digit number.

If you know the agency number, enter it here and click ADD

If you don’t know the agency number, search for it here

To add a relationship, follow these steps: �

  1. Navigate to the "Search By" section.
  2. Select the Agency Type from the drop down menu.
  3. Select the Relationship Type from the drop down menu.
  4. Type in only the beginning of the name of the public school district, private school, or educator preparation program in the Agency Name field. Partial searches are highly recommended.
  5. Click the "Search" button.
  6. All agencies that match the partial name are displayed in the list below, including an assigned agency number.
  7. Type the 8 digit agency number from the desired agency in the Agency Number field in the "Related Licenses" section.
  8. Click the "Add" button to add the relationship.
  9. Repeat steps 1-8 if a second relationship is required.

You know that you have successfully added a relationship when you have the option to “DELETE” that relation. Now you can move to the next page.

The page will refresh once the uploading process is completed. Once attached you can View or Remove each file as needed. Repeat the same three steps to attach the PI-1622-Sub form.
