Double check that your Date of Birth is correct

Click Add IF you have more than one degree
NOTE: Click ADD button (above) IF you have worked in a paid position in a PK-12 school. Do not report student teaching experience. If no experience to report, click Next.
Click Add IF you have PK-12 work experience
The agency number is a district’s 4-digit LEA number with four 0’s added onto the end, to make an 8-digit number.
If you know the agency number, enter it here and click ADD
If you don’t know the agency number, search for it here
To add a relationship, follow these steps: �
You know that you have successfully added a relationship when you have the option to “DELETE” that relation. Now you can move to the next page.
To confirm what attachments are needed, open drop down

Upload transcripts in this slot
Upload the PI-1622-Sub form in this slot
Upload evidence of successfully completing district-selected substitute training here (if needed)
Select an attachment type from the list and then locate your file with the "Browse" or "Choose File" button.
Notes are optional. Click "Attach File" to upload each file with your transaction
The page will refresh once the uploading process is completed. Once attached you can View or Remove each file as needed. Repeat the same three steps to attach the PI-1622-Sub form.
Confirm both attachments have been uploaded before clicking “Next”
The application summary shows what you’ve entered on each page of the application. THIS IS YOUR LAST CHANCE TO CORRECT ANY ERRORS. READ THE SUMMARY CAREFULLY.
Click “EDIT” if you need to change any information in your summary
Once finished editing, click “Save/Return to Summary”
Once you are satisfied all information is accurate, submit the application at the bottom of the summary
Answer this attestation and click “SUBMIT” again
