Welcome to Simmons!

These slides will review the items you need to submit/complete as a new student. You can also review the requirements on the Simmons website and/or watch our how-to video.

  • Immunizations and Forms
    • ALL full-time students under age 30 who will take any classes in person
    • ALL international students
    • ALL health science students of any age who will take any classes in person

  • Health Insurance Information
    • All students enrolled ¾ time or more must either enroll in the Simmons Student Insurance Plan or be enrolled in an insurance plan with qualifying, comparable coverage

Deadline and How to Submit

  • You must use the Student Health Portal to submit your health information. Please do not mail, fax, or email documents.
  • If you have any questions about these health requirements or using the Student Health Portal, please contact the Health Center healthcenter@simmons.edu.
  • New fall students should submit their information by August 1st, 2025. New spring students should submit their information by January 5th, 2026. Please note that failure to meet these requirements may impede your ability to register for classes, so it is important to submit your information in a timely manner.

Undergraduate Students

New undergraduate students are required to submit the following information on the Student Health Portal:

  • Consent for Treatment
  • Health History
  • Tuberculosis Risk Assessment
  • Health Insurance Information
  • Required Immunizations

Graduate Students

New graduate students are required to submit the following information on the Student Health Portal:

  • Tuberculosis Risk Assessment
  • Health Insurance Information
  • Required immunizations

Tips and Things to Note

  • You must use the Student Health Portal to submit your health information. Please do not mail, fax, or email documents.
  • Start now- contact your provider to make sure you aren’t missing any immunizations!
  • You must enter the dates of your immunizations AND upload an immunization record listing the doses and dates of each immunization- if you don’t complete both steps, we cannot verify your record.
  • We accept your most recent official immunization record from your healthcare provider, official medical records, or records from another academic institution OR you can have your healthcare provider complete and sign our form.

Tips and Things to Note

  • If your records are incomplete, but you believe you have received the required immunizations, you can obtain titers (blood tests) to show your immunity and submit a record of those tests.
  • If you’re having trouble getting an appointment with your provider, you can get immunizations and titers at many urgent care clinics and pharmacies.
  • If you received your immunizations in the United States, you may be able to obtain records from your state’s registry.

Student Health Insurance

All students enrolled at ¾ time or more are required to be covered by health insurance. There are three options for insurance coverage:

  • Simmons University Student Health Insurance Plan
  • Coverage through family’s health insurance plan (through age 26)
  • Individual private or state-sponsored insurance plan

The deadline to waive the student plan is September 15th, 2025. After that date, all students who have not waived will be automatically enrolled. Students may also enroll prior to September 15th if they would like to receive their insurance card/information before the semester begins.

Comparable Coverage Checklist

Planning to waive the student insurance plan? Use this checklist to review your coverage before you waive. Students who waive with inadequate coverage often find themselves unable to access needed care or are charged for expensive procedures that their insurance doesn’t cover.
