TESTING FOR TUBERCULOSIS (TB)

• No student shall be permitted to attend any public school, unless they have on file a report of a TB skin test result according to 10 GCA § 3329 Testing for TB

• TB skin test is required for all DOE students who transition from elementary to middle school (6TH grade) and middle school to high school (9ᵗʰ grade), and any student who enrolls for the first time in GDOE irrespective of their grade level

• IMMUNIZATION

Minimum immunization REQUIRED for school attendance:

• Diptheria, Tetanus, Pertussis (DTaP, DTP, DT, Td) = 4-5 doses: MIN. REQ. - at least one dose after the 4ᵗʰ birthday
• Tetanus Booster (Tdap, TD) = 1 dose: REQ’T. – one dose required if 10 years elapsed since last DTaP, DTP, or DT)
• Polio (OPV, IPV) = 3-4 doses: MIN. REQ’T. – one dose
• Measles, Mumps, Rubella = 2 doses: MIN. REQ’T. – two doses
• Polio (OPV, IPV) = 3-4 doses: MIN. REQ’T. – one dose
• Hepatitis B = 3 doses: MIN. REQ’T. – one dose

EMERGENCY INFORMATION AND HEALTH FORM (EIHF)

• BP 337 requires that the student EIHF be completed annually and updated as information changes.

• Form will be provided by the school at the beginning of each school year and it is the parent’s/guardian’s responsibility to inform the school of any changes ASAP; if family members are not listed, they will not be allowed to pick up child

​

• The homeroom teachers are responsible for distributing and collecting the forms as well as maintaining a checklist to ensure that all forms are collected from each student the first week of school.

OTHER HEALTH MATTERS

IMPORTANT MATTERS THAT NEED TO BE COMMUNICATED TO THE NURSE:

• ALLERGY
• ensure that information is specified in the EIHF and PE of child

• MEDICATIONS
• Parents need to see nurse to obtain and complete the Medication Consent Form, and to turn in child’s medication for school.
• Students are not allowed to transport medications, unless specified by the doctor in his / her medication consent form (self administration such as Albuterol Inhalers)
• No medication shall be administered to the child at the school site, until the completed consent form has been submitted.
