What is it?

First of all, you see many different terms for this in the news. Like coronavirus or Severe acute respiratory syndrome coronavirus 2 or coronavirus disease 2019 (COVID-19). I think it is important just to clarify that they may be different for medical professionals, but for the most part, you can just think of these as the same thing.

Second of all, it is important to know that this is a virus just like many other viruses. And that is a good thing, because we already know a lot about it. But it may also make it seem not very special. One thing you hear a lot is that it the common cold is a coronavirus, so it can’t be that different. Or maybe you hear that it is just like the flu in most people.

Why is it different:

Our bodies are used to fighting off colds and the flu. But our bodies have never seen something like this virus, so our bodies may have a hard time fighting it off.

It is also easier to transmit to eachother than the flu. On average, most people will give the virus to 2 or 3 other people while they are sick (R0 2 - 2.5 JAMA). Then each of those people will give it to 2 or 3 more people, and so on. This is different than the flu, which is usually only transmitted to one other person (R0 1.3). So it spread faster than the flu.

The illnesses caused by the virus can be very severe, especially in the elderly. The flu can also kill people but it is more rare. The flu kills one in a thousand people. This coronavirus kills 1 out of every 40 people.

Unlike the flu, we really don’t have any medications like tamiflu or vaccinations like the flu vaccine that can be used very successfuly to treat people with this.

Think of a subway car where everyone is seated. Not a jammed packed subway car, but just a car where people are sitting in every available seat. That’s about 50 people. You can think of this next time you are on a subway car, which I hope you do not have to do soon or often.

If all 50 people on that subway car get the virus, then 40 of those people -- mostly the young people who are under 50 years old will walk around just like they had the flu. They won’t even go to the hospital.

10 people though will get really sick. They will have to go into the hospital for something like pneumonia.

2 of those people in the hospital will get so sick they will have to go to the ICU, or the intensive care unit. They will have to be hooked up to a breathing machine called a ventilator and they will have to fight for their lives.

Of those two people, only one person will survive. The other person will not make it.

That can be scary, but it is important to remember that even if you get it, you have a very good chance of just having a mild disease.

It is also really important to know that most people who are at risk of severe illness ar those over the age of 60 years old. Those people are at highest risk of something bad happening, and are the people we most need to protect!

Sometimes it seems like the news on coronavirus changes everyday. You might hear one statistic one day and another statistic another day. Why is it all so confusing?

It is confusing because it spreads very very fast from country to country, and so the number change very quickly. It is also confusing because these numbers are calculated in different ways. This is the process of science and it is messy.

Countries have very different responses. As an example, it is estimated that the virus killed as many as 1 in every 25 people in China, but only 1 in every 200 in South Korea. Those numbers are very, very different.

In the US, we have not had a test. So we have not known where it is and where it is not. We simply do not know who has it. Each day, we get more tests and more numbers and we understand our risk more and more.

And the last part is that we made a lot of effort to keep it in China, but now we are not doing that. We are just trying to make sure that as few people get it as possible.

Lastly we did not fund government programs to do research or disease tracking. And we made mistakes about test kits. It is not time to point fingers, but we made many mistakes as a country.

Because everything changes day to day, advice from yesterday is no longer relevant today.

Graphs can be tricky, and you may not be familiar with them. That is ok. But I think this one may help you.

This is the number of cases over time in China. The numbers don’t matter so much. Each bar is one day. So over on the left there were only a couple of people walking around with coronavirus. Noone was worried. Noone was even that sick.

But then the number started to grow, and when it grows, it can grow really, really fast. We call this exponential. It shoots up. Think of each line like a train car. On the first day, it was just one train car full of people, but 20 days later, it was like every single person riding on all train cars in all of New York City.

But it is actually much, much worse than that. Because the grey bars are not the cases we knew about. The yellow bars are the cases that we known about.

Because it spreads so fast, and grows so quickly, So for example, in New York, it was 420 on Friday, and 520 the very next day, and then 600 something. But those are the yellow bars, not the grey bars.

Today they tell you there are 1000 cases in New York. But that is only what they know about. There maybe 10,000 cases in New York.

But it is still worse than that, because tomorrow they will tell you that there are 2000 cases in New York. But that is only because the yellow bars don’t seem to move as much as the gray bars. The actual number of cases might be jumping to 20,000 cases. And when they say there are 4,000 or 5,000 cases, there may be something like 50,000 or 100,000. We simply have no idea.

This is what everyone is scared about. Not the number of cases in the news, but the total number of people walking around infected.

This is the good news. Even though the number of cases in yellow that we know about will rise and rise and rise for the coming 2 weeks, we can make changes now so that the actual number of new cases, which is in grey starts to fall.

So What China did is that they built new hospitals and totally locked down the province of 60 million people. They put tremendous effort into this and they didn’t let anyone out of their homes. And by doing that, the number of new cases started to fall. And the new cases began to fall and fall and fall in grey, even when the number reported in the news continued to go up and up and up. BUt eventually, both came down. And now they have it under control.

The point of all of this is to say we should be very, very worried. We need to stop interacting with others at all costs. Stay home if you are not a healthcare provider or doing an essential function. And protect our elders. We can do this, but we need to make changes now. Today. And then, in a couple months, things will get better.

In other words, everyone has to stay home now. If you do go out, you need to use precautions like hand sanitizer and gloves.

In places like NYC where there is ongoing transmission, we can assume that anyone leaving their home to work will be at risk of being exposed to the virus. As mentioned, hospital workers (theoretically the most at risk) are not wearing respirators themselves because the likelihood of transmission is the same as being on the subway at this point.

But we know that not every job can shut down right now, so we need to think through and implement every possible trick in the book to support stopping the spread of illness and prevent exposure. Remember at this point it is a two way street where not only could we be exposed but we can also expose others, since many of us could be carrying germs without showing symptoms yet.

It’s important to understand when thinking about controlling this or any hazard that some methods are more effective than others. Using mask or gloves are going to provide limited protection from contacting or spreading germs because exposure is already happening at that point. It is much more effective to practice hand washing, good hygiene, and maintaining distance from sick or potentially sick people (and at this point, we should assume that everyone has the potential to be sick). Even better to eliminate the chance of exposure by encouraging workers to work remotely, or offer sick pay so people feel like that can take time off when sick.

So let's quickly talk through some other examples of how we can reduce or minimize the risk of exposure to this danger. So if we cannot change what we’re doing to eliminate exposure completely, we want to try and isolate it physically. For workers, who are exposed to the public can be using sneeze guards, or healthcare workers treating sick or at risk folks can be utilizing high efficiency air filtration that will contain most germs, or isolation rooms.

Next we want to consider things we’re going to DO as a administratively to limit exposure. This is where things like sick pay go, which is encourages workers to stay at home. This is where we see businesses establishing new policies for sick workers, cleaning procedures, limitations to the number of people gathered, etc and communicating that to workers and clients. A wonderful way we are effectively eliminating the risk right now is by holding this gathering over an online telecommunication platform, as many groups are creatively taking up as well.

Administrative controls examples:

Encouraging sick workers to stay at home.
Minimizing contact among workers, clients, and customers by replacing face-to-face meetings with virtual communications and implementing telework if feasible.
Establishing alternating days or extra shifts that reduce the total number of employees in a facility at a given time, allowing them to maintain distance from one another while maintaining a full onsite work week.
Developing emergency communications plans, including a forum for answering workers’ concerns and internet-based communications, if feasible.
Providing workers with up-to-date education and training on COVID-19 risk factors and protective behaviors (e.g., cough etiquette and care of PPE).
Training workers who need to use protecting clothing and equipment how to put it on, use/wear it, and take it off correctly, including in the context of their current and potential duties. Training material should be easy to understand and available in the appropriate language and literacy level for all workers.

We also want to think about what we’re going to DO to establish safe work procedures to reduce the duration, frequency, or intensity of exposure. This can be things like hand washing, use of sanitizer, keeping separate equipment or any other practice you can think of that would limit the spread of germs.

Washing hands is better than hand sanitizer; use a little hand-sanitizer to wash your phone — it dries quickly so should not cause moisture to damage the phone; use of regular gloves (like winter gloves / gardening gloves) for going on subways etc — so as to keep your hands cleans without using sanitizer and which may reduce amount of times you touch your face; and also the onion / foul smelling hands trick. These are just very practical and easy to do.

Examples of safe work practices for SARS-CoV-2 include:

Providing resources and a work environment that promotes personal hygiene. For example, provide tissues, no-touch trash cans, hand soap, alcohol-based hand rubs containing at least 60 percent alcohol, disinfectants, and disposable towels for workers to clean their work surfaces.
Requiring regular hand washing or using of alcohol-based hand rubs. Workers should always wash hands when they are visibly soiled and after removing any PPE.

Minimally, businesses will need to establish a change in policies to implement any of the things we’ve discussed so far. One friendly HR legal reminder to be careful that new policies do not to discriminate against any protected class; for instance if work is ongoing you can’t require workers to not work because of a risk factor like age, even if well intentioned. Understand that if pay isn’t offered to workers, they may resist opting to stay home for fear of loss of income.

Moments like this give us opportunities to see where the systems we set up fare against a crisis. Can the work be done over telecommunications, and if so, are workers equipped to do that? How can the business compensate workers who lose wages because of cancelled jobs? How can the business compensate workers who miss work because they are sick? How can the business compensate for lost wages because of cancellations?

Use the chat to throw out any steps your business has made or is planning to make in regards to a contingency plan.

As this gets worse here in NYC, less work will being considered so “essential” to risk further exposure to the virus and we’ll need to deal with those changes or losses many of us might already be feeling. And so we start thinking of contingency plans and further support we’re going to need.

Now is the time to check in with each other your in business with, if you haven’t already. Consider this impact this is having on your work or will have, not just this week, but in two weeks, or next month. Identify what you will need under these circumstances if the business is going continue to operate. Find out what contracts or agreements need to be adjusted in order to accommodate these circumstances. If you have liability insurance, find out if the policy would cover losses caused by a natural disaster of this kind.

Most importantly is for us to gather and organize with each other and our communities at this time to deal with this issue that is clearly bigger than any of us individually. The reason we gathered this call, to share best practices and start a conversation around the support folks are needing.
