PARTICIPATION/CONSENT FORM

CLASS ATTENDANCE

  • Student-Athletes must attend first period classes in order to be eligible for sports, clubs & activities
  • The only exception is a doctors appointment. Student athlete must return to school with a doctors note that is brought to the athletic director to seek administrative approval.
  • Student-Athletes will be ineligible for sports, clubs & activities if:

1. They are absent from school

2. They arrive late to school or miss first period without a doctors note

4. They are dismissed from school by the school nurse

ATHLETIC ATTENDANCE

  1. After 3 unexcused absences the coaching staff will have a conversation with the student athlete providing a warning and expectations on how to maintain membership on their team.

  1. After 4 unexcused absences the coaching staff will inform the athletic director that they have had a conversation with the student athlete providing a warning along with expectations on how to maintain membership on their team In addition, a meeting with the student athlete, parent, coaches and athletic director will be scheduled to determine membership on the team.

  1. Each time a student athlete, on a team sport, has missed a game/practice due to an unexcused absence that student athlete will sit out of their next scheduled competition for 30% of the overall game time.

  1. Each time a student athlete, on an individual sport, has missed a combined total of 2 meets/matches/practices due to an unexcused absence that student athlete will not participate in the next competition.

  1. Each student athlete will receive one mental health day per team, each season that can be taken as needed no questions asked. Coaches must be informed in advance that the student athlete is taking their one mental health day.

TRANSPORTATION

  • Student-Athletes must travel to and from athletic competitions on school provided transportation unless their parents/guardian are picking them up at away sites.
  • Any other arrangement requires a permission slip signed and completed by parents/guardian that must be submitted in advance & must be approved by administration.
  • Permission slips can be picked up in the athletic office/website
  • Seniors are not permitted to drive teammates at anytime
