What is a Family Care Plan ?

  • Governed and required by AFI 36-2908 and DoDI 1342.19
  • Establishes an advanced plan for the CARE of dependent family members of Service members during a period of absences such as:

-contingency operations

-severe weather evacuations

-exercises

-operational requirements

-overseas assignments to dependent-restricted areas

  • Documented on AF Form 357
  • Failure to produce a Family Care Plan (FCP) may be subject to disciplinary or admin action IAW DoDIs 1332.14 and 1332.30 (ref. (f) and (g))

Who will have a FCP?

ALL AF MEMBERS WITH FAMILIES WILL HAVE A PLAN !

Documented on AF FORM 357:

    • Single Parents
    • Dual-Member couples with dependents
    • Military members with civilian spouses who have unique family situations
    • Primarily responsible for dependent family members

What does a FCP include?

​

A FCP includes provisions for the Welfare of the Family Members:

    • Legal (Power of Attorney)
    • Medical, Educational, Financial, Religious, and Physical Well-being support needed
    • Housing, Transportation, Food (logistical support)
    • Base access for non-military family members

Designates arrangements for the member’s family care and should be specific enough to cover all situations while providing a smooth, rapid transfer of responsibilities.

Member Responsibility

​

  • Must notify CCF or CC when there is a change in family status
  • Develop FCP IAW AFI 36-2908 and DoDI 1342.19
  • Designate Short and Long Term caregivers- ensure family members are aware of roles and accept the responsibility
  • Complete and return signed AF Form 357 with POA attached to CCF within 60 days (90 days for non-active duty)
  • Review and recertify on an annual basis
  • Provide any other pertinent information as needed as an attachment to AF FORM 357 (day care, schools, special needs info etc.)
