STEP BY STEP

  • Click the CREATE button, click More, click purple Google Forms.
  • Type the Title of your form (decide on nomenclature style e.g. 1703 Museum Field Trip) and add Form description. At the top,
    • click PALETTE icon (unless default is okay). Click IMAGE icon for themes or photos (great for differentiation or personalization).
    • click SEND icon to collect usernames OR to email OR to add collaborators and choose access.
    • click 3 DOTS icon for Preview OR Settings options (e.g. GENERAL - collect email address) OR PRESENTATION (e.g. Show progress bar) OR QUIZZES options (see ____ below).
    • click your profile picture OR initial to switch Google accounts.

  • Whenever you see at the top
    • the left arrow, click to RETURN to previous option.
    • the pencil, click to continue to EDIT
  • Under QUESTIONS tab,
    • enter first question under Question Title (replace "Untitled Question" with “First Name” or “Class Period” or “Select which quotation to explain and its location”).
    • add any additional explanation in the Help Text box (e.g. Cite page numbers as "(15)," not "page 15.")

  • Select Question Type (9 options), which show up differently as lines of text or charts or graphs in the Summary of Responses.
    • Multiple Choice: allows one choice from list, including images clicking "Go to page based on answer" creates a "branching" form, can be a mini-lesson or a "password"
    • Checkboxes: allows multiple answers
    • Dropdown: allows one choice from a drop down menu
    • Short answer: for up to a few sentences (e.g. Firstname, Lastname, “what3words”)
    • Paragraph: for longer answers
    • Linear scale: for labelled ratings from 1 to 10 (or fewer) on a scale
    • Multiple choice grid: for ranking (such as top choices for electives) or non-numerical descriptions
    • Date OR Time: self-explanatory, common questions

Click Required Question if applicable, also click 3 DOTS icon to:

Duplicate a question OR remove it

Add additional description (such as “Must be a number with decimal” or “must be a URL”

Data validation: SUPER POWERFUL (options depend on question type)

Multiple choice OR Dropdown:

Use “Go to section based on answer” for “password” or differentiation based on right answer

Use “Shuffle option order” for quizzes on adjacent computers -- sneaky!

Checkboxes: use “Select at least/most/exactly” to force or limit number of choices

Short answer:

Number OR Text OR Length OR : maximum OR Regular expression

Paragraph: length OR regular expression (same as short answer)

Multiple choice grid: limit per column OR shuffle row order

  • Under RESPONSES tab,
    • Click green Google Sheets icon to choose destination for responses (usual NEW)
    • Click 3 DOTS icon for additional options (email notifications OR download as .csv file OR print all responses OR delete all responses such as for new school year)
  • Bottom of Form:
    • Click PLUS icon to add a new question.
    • Click DOUBLE T icon to add a new title/label (vs. question or section)
    • Click IMAGE icon to add an image at the same level as a question or title.
    • Click PLAY icon to add a video at the same level as a question or title.
    • Click EQUAL icon to add a new section.
