Welcome to EJHS

Philosophy

The goal of our athletic programs shall be the development of sportsmanship, the teaching of fundamental skills, and the opportunity for participation. Fair playing time for participants will be an objective based on the effort, attitude, and ability exhibited by each individual participant.

Required Athletic Forms

  • All forms should be filled out by the Athlete and Parent/Guardian
  • Physical filled out by parent/guardian and physician/medical provider (Available through the Evergreen Cares Clinic at East Evergreen Elementary)
  • Parent Consent and Medical Release Form (Must fill out Part G with insurance provider and policy number)
  • Participant Contract
  • Concussion Education and Compliance Form

  • Away Contests: Travel together on the bus
  • Parent Sign-Out Sheet: Must be signed in person after the contest
  • Third-Party Release Form: Must be completed 24 hours in advance
