Why RED Exists

Rapid Engagement Delivery (RED) and its history over the last several years

There is a strong correlation between the chronically homeless and high utilizers of the health care system

To assist Monroe County residents with obtaining Temporary Assistance (TA) to secure housing, food, clothing and other basic necessities

Medicaid and access to healthcare

Streamline and the entire DHS Temporary Assistance Process

What Are The Requirements

Monroe County Resident

Eligible for Temporary Assistance

Between the ages of 18 and 64 years old

Mental Health and/or Substance Abuse barrier to employment

Willing to participate in treatment

Any combination of three or more TA Case Closures, TA Case Denials, or DHS approved Emergency Housing Placements within the past 12 months

One page referral and two consents needed

Referral and Consents reviewed for criteria

Referral source updated

If the member meets criteria – RED Team will reach out to further screen member.

If the member NOT meet criteria – only the referral source is updated and given helpful information

Workflow

Initial approval email from Prissilla Duerte- Establishes Starting Point in RED Process

Jonathan Martinez and Brian Hooker will establish the best means of communication with new member.

At initial meeting, gaps in care are identified and a plan is made to connect member to additional services.

Prissilla will conduct the TA Interview. Determination of a ‘track” is determined at this time.

Member is mailed information on paperwork needed to open TA Case, Jon and Brian are also included

As needed, support is given to ensure attendance to CASAC, Evaluation for SUD

Member, RED, and care entire team work together to get benefits open and keep open.

Ongoing coordination of care providers while TA Case is active
