New Features:

504 Status - Eligible, Ineligible, Monitor, Withdrawn
Parent Meeting Notification Date – Date parents are notified of the meeting
Accommodation Information: Does the student currently require accommodations? Addresses students who are eligible for Section 504 but do not require accommodations, typically due to the positive effects of mitigating measures.

Revised Wording:

“Student history and any evaluative data considered at meeting” has been changed to “Student history and any other relevant information not noted above”

We now have two new headings and a new format for choosing accommodations.

First we have…

System Wide and General/Classroom Assessment Accommodations which allows you to choose system-wide and general classroom assessment accommodations. Remember, students who have access to accommodations for system-wide assessments should also have access to those same accommodations throughout the school year when taking assessments in class.

Next we have…

General Classroom Accommodations which allows you to choose daily classroom accommodations as well as accommodations for other school settings.

A couple features have been removed from the template:

“Other accommodations not listed above” and “Summary of meeting and comments not noted above” are no longer featured in the template. The Signature Section has a different format on the printed 504 Accommodation Plan Report and now includes a statement that states that parents agree to the 504 Accommodation Plan and have received a copy of their Rights. The 504 Accommodation Plan now prints in color.
