Foundation Models and Electronic Health Records

RQ: Do FMs trained on a specific patient population generalize effectively to fundamentally different healthcare systems?

  • Less labelled data for downstream tasks
  • Little or no feature engineering
  • Embeddings encode the pretraining population's disease mix, guidelines and care practices
  • Interoperability: Different coding systems and data models across site

Discussion and conclusions

A US-pretrained FM matches count-based models on Italian data, with less feature engineering

What we found

  • A US-pretrained FM transfers to Italian data
  • Performance similar to count-based models, with less feature engineering
  • Shared standards (OMOP, MEDS) make the transfer possible

Limitation

  • Patient history covers Maugeri admissions only; stays and visits elsewhere are missing
  • Past history seems to add little for the current stay

Next steps

  • Predict new clinical tasks
  • Explainability (XAI) of embedding-based predictions
