Introduction

  • Shelf life is the amount of time a product can be stored before it's no longer fit for its intended use.
  • The expiration of sterile packs depends primarily on the packaging, storage conditions, and whether the facility follows time-based or event-related sterility guidelines.
  • Expiry dates on surgical packs are assigned based on the guarantee of sterility. The method of sterilization establishes the type of packaging material that may be used.

Guidelines for shelf life of sterile packs:

  • Some people may view shelf life and expiration dates as the same thing but there’s an important difference.
  • A sterile item does not have a set expiration date but is considered non-sterile if a contaminating "event" occurs.
  • An item is considered sterile on the basis of handling of the package, integrity, type, and configuration of packaging material and Conditions of storage.
  • Sterilized surgical/instrument packs should be labeled with the following information as a minimum, Autoclave tape and/or indicators, date pack was sterilized and expiry date.

Event-related shelf life

  • Definition: A pack remains sterile indefinitely as long as its integrity is maintained regardless of expiry date.
  • This means that a pack remains sterile indefinitely as long as its packaging is intact, it's stored in a clean, closed, and moisture-free environment, and it hasn't been contaminated.
  • Storage conditions are established to maintain the integrity of the package.

Factors that affect shelf life

  • Shelf life is affected by the permeability and density of the material, the type of closure used, and the method by which the package is handled.
  • An item is no longer considered sterile after an accidental puncture, tear, or rupture of the package.
  • Paper may become brittle and crack.
  • An accidental wetness, presence of dust, dirt, destruction by vermin and squeezing or crushing of package will render the pack unsterile.
  • A pack must be re-sterilized regardless of its "expiration date".

Time-based shelf life

  • The shelf life of sterile surgical packs is event-based, not time-based
  • It depends on maintaining the integrity of the sterile barrier.
  • While traditionally set times applied (e.g., 30 days for paper-wrapped, up to a year for peel packs).
  • These are now considered outdated
  • Current practice focuses on ensuring the integrity of the packaging
  • Integrity maintained from the sterilization date to the point of use.

Time-based sterility (TBR)

  • The manufacturer’s expiration date marked on a sterile pack or medical supply item shall take precedence
  • The shelf-life/expiration date for medical/surgical/instrument packs or supplies that are sterilized/re-sterilized in-house is determined by the type of wrapper/packaging material.

Key Factors Determining Sterility

Packaging Material:

Paper-based, cloth, or modern paper/plastic peel packs offer varying levels of barrier protection. 

  • Storage Conditions:

Storing packs in a controlled environment free from moisture, dust, and damage is crucial. 

  • Handling and Movement:

Each time a pack is moved or handled, the risk of compromising its sterility increases. 

  • Maintenance of the Sterile Barrier:

The packaging must remain sealed and undamaged until the moment it is used to prevent recontamination

Shelf life of packages

  • Muslin or paper wraps: Packs wrapped in double-layered muslin or paper generally have a sterile shelf life of about 30 days. This can sometimes be extended to six months if stored in closed cabinets.
  • Double-wrapped packs with heat-sealed overwrap: Adding a sealed, 3-mil polyethylene overwrap to muslin or paper-wrapped packs extends the shelf life indefinitely, provided the overwrapped package remains intact.
  • Heat-sealed plastic and paper pouches: These "peel packs" have a longer sterile shelf life, typically considered to be up to one year, if stored correctly and the seal is unbroken.

Storage of sterile packs

  • After sterilization and immediately aerate
  • Protect from extreme temperatures
  • Ventilating and air-conditioning systems with filtered
  • Temperature below 75° F (24° C) and a relative humidity between 30% and 70%.
  • Four air exchanges per hour are recommended
  • Peel pouches should be stored on their sides to minimize the pressure
  • Don’t stack on top of them

  • For open shelving, the highest shelf should be at least 18 inches (46 cm) below the ceiling or sprinkler heads
  • 8 to 10 inches (20-25 cm) above the floor.
  • At least 2 inches away from the wall.
  • Closed cupboards are preferred.
  • Sterile storage areas should have controlled traffic patterns that do not pass by open shelving storage.

Conclusion

  • Items on open shelves have a high likelihood of contamination as they are exposed to the outside air and dust particles.
  • If they’re on closed shelving or in drawers, the likelihood of contamination is significantly lower, so they may have longer shelf life sterility.
  • Ultimately, the final decision on the shelf-life of reprocessed instruments rests with each healthcare facility, based on its internal risk assessment.
  • Many hospitals are moving toward event-related sterility, which dictates that an item remains sterile indefinitely unless the packaging is compromised. 
  • Local policies on sterile storage should be developed by an infection prevention and control (IPAC) committee and clearly communicated to staff to address the handling and storage of all stored sterile supplies.

The "When in doubt, resterilize" rule:

  • If you are unsure about the integrity of a pack or storage conditions, it's safest to re-sterilize it before use
