Bharavahana carrying heavy loads over the shoulder will cause vata prakopa and deformity in the joint capsule. This leads to disease formation.

Balawat vigraha wrestling with a person who is more powerful will cause aghata to the amsapradesha and vataprakopa takes place.

Dukkha shayya improper posture that creates a great amount of more pressure on the amsasandhi will disturb the muscular integrity, and provokes vata. This manifests the disease.

Frozen shoulder

  • Characterized by stiffness, pain and limited range of movements
  • Pain usually worsen at night
  • It most commonly affects people between the ages of 40-60, and occurs in women more aften than men.
  • People with diabetes are more likely to develop the condition and it also takes longer to recover.
  • This usually occurs over a 2 year period

Pathology

  • The shoulder is a ball and socket joint which allows it to move in more directs that any other joints in the body. There is a capsule around the joint which is made up of soft tissue fibres. In a frozen shoulder, this capsule becomes contracted and forms bands of scar tissue called adhesions. This causes stiffness and aften severe pain in the shoulder joint.

  • Pain in the outer region of the shoulder that can radiate intothe outer upper arm
  • Worse at night and in cold environment
  • Inability to sleep on the affected shoulder

Phases

  • Phase 1: freezing phase: the shoulder gradually becomes more painful over a period of time. With pain, theshoulder starts becoming stiffer.
  • Phase 2: frozen phase: the shoulder pain improves a little during this phase. However, shoulder movements will be extremely limited and performing daily tasks such as dressing or grooming will be painful and difficult.
  • Phase 3: thawing phase: the shoulder pain will gradually alleviate and diminish during this phase.it will also be easier to move the shoulder as the stiffness improves while your shoulder is “thawing”.

  • Abhyanga : the veerya of the drug used for abhyanga gets digested with the help of sthanika bhrajaka pitta and enters into the srotas and starts its action.

taila used for abhyanga after entering into the body nourishes the body tissue, give strength.

  • Swedana: it does the dilatation of the blood vessels thus by improving blood circulation.hence it is very beneficial in conditions where shola, stamba and samkoch are present.

swedana karma like pinda sweda, bashpa sweda, nadi sweda and avagaha sweda are mentioned which alleviates vata dosha.

  • Nasya karma: it gives strength to neck, shouldes, chest and increase vision.

  • Apabahuka is a disease caused by kupita vata dosha, localizing around the amsa pradesha causing the shoshana of amsa sandhi.

  • There by leading to akunchana of sira at that site and giving rise to bahupraspandaharatwam which means restricted movement of bahu.

  • Vyanavata is responsible for all types of motor functions namely prasarana, aakunchana, vinamana etc. which is affected in apabahuka.

  • The kapha present as lubricating material within the sandhi aids in the movements such as prasarana, aakunchana etc.

  • Shoshana of kapha leads to impaired range of movements.

  • In Apabahuka Vatahara and Brimhana Dravyas are useful in the form of Nasya and Snehapana. As aushadha kala of Vyana and Udanavata is adhobhakta, Uttarabhaktika Snehapana do the shamana of vyanavata prakopa and subsides the disease.

  • Brumhana is the one, which is done or used for the nourishment. In the present context of apabahuka the vitiated vata due to its rukshadi qualities does the shoshana of shleshaka kapha that is present in the amsa sandhi. To subcide this rooksha quality and to normalize the shleshaka kapha qualities, brumhana snehapana is advisable.
