  • OAP is Cigna’s broadest network.
  • Local Plus is Cigna’s narrow network. It removes TriStar doctors and facilities from in-network.
  • Williamson County claims are $500 per employee per month or $6000 per year higher in OAP than Local Plus.

Deductible Plan Versus HSA Plan

  • The Deductible plan are more expensive than the HSA.
  • The Deductible plans are $300 to $400 more expensive on a premium equivalency basis than the HSA per month, or $3600 to $4800 more per year. Even adding in the $500 annual HSA contribution, the Deductible plan is more expensive than the HSA option.

Plan Design Change: ER and Urgent Care

  • ER costs are 25% higher than the national average of other plans like Williamson County.
  • Increase the ER Additional Copay from $155 to $300 or $500 on the Deductible Plans. The Copay will be waived if admitted to the hospital.
  • Add a true $50 copay for Urgent Care on the Deductible Plan.
  • These changes should encourage more Urgent Care and fewer ER visits. This should curb rising ER costs and reduce overall claims costs.

Recommendations

  • Change the Employee Contribution to $15/$30/$60.
  • Change the HSA and Deductible plans Deductible by $300 EE/$600 Fam and Out-of-Pocket by $1600/EE and $3200/Fam saving $4.8 million.
  • Change the ER additional copay to $300 and add a $50 Urgent Care copay, encouraging fewer ER visits.
