Nonpublic School Consultation Participation Form

This meeting is intended to discuss your participation in Title I and/or Title II grant programs for the FY 2027 school year as part of the Non-Public School Participation Grant Application process.

Non-attendance or no response to this invitation will confirm your non-participation in the Title Programs.

To determine Title I eligibility, the nonpublic school will need to provide the following:

· Names and the address of student(s) who reside within the boundaries of the school district

· Registration of student(s)

· Grade level of student(s)

· Low-income status of the student(s) – Verification of Low-income status of student(s)

· Report Card of student(s)

· Assessments of student(s)
