In 18 year old cardiac patient undergoes valve replacement surgery in one of the reputed hospitals. The surgery was successful and the patient recovers well following the surgery. On discharge the Cardiac surgeon prescribes Tab Warfarin 5mg and asks the patient to repeat an INR test 15 days later and based on the report the dose of the drug will be altered. Patient goes back home, continues to take the tablet and also attends college. As prescribed 15 days later the patient gets a blood test done, calls up the doctor and tells the report over the phone. Now the doctor advises to take 2.5mg but the patient understands it as 2 1 ⁄ 2 tablets. Now the patient starts to take 12.5mg of warfarin instead of 2.5mg”.

Imagine what can happen if the patient takes 12.5mg. 2 days later the patient develops sudden bleeding symptoms and it becomes life threatening. Patient was immediately rushed to the hospital. The treating doctor was shocked looking at the patient’s condition and asked the relatives how it happened. The relatives explain that the patient developed these symptoms after the change in treatment i.e, the patient started to take. The medical team puts their maximum effort to save the life of the patient but it goes in vain.
