  • Resolved, that the Academy educate pediatricians on the Comfort, Ask, Relax, Distract (CARD) system which would include evidence-based research regarding its usefulness, and disseminate information regarding the availability of the CARD system, and be it further
  • Resolved, that the Academy encourage all pediatricians and their teams to use the Comfort, Ask, Relax, Distract (CARD) system to decrease needle stick pain and anxiety/fear.

1. My idea for a resolution (in 2016) …

  • Resolve… current shortage of home care nurses
  • Problem: Families of children with medical complexity and technology dependence cannot access home nursing services
  • Why me? I see a disproportionate number of children with medical complexity who have technology dependence
  • What next? Home care agencies tell me that the “pay” is not high enough so that home care nurses are leaving for “private industry” in droves; Medicaid reimbursement too low (equity issue)

2. Research the topic…

  • 2011: that the Academy advocate that home care services such as bilirubin measurement, vital sign monitoring and weight checks be available seven days a week
  • 2017: that the Academy champion efforts for trainees and pediatricians who will be providing care for this growing population of children; and promote care models that address the need for integrated coordinated services which may include interdisciplinary clinics and telehealth models that minimize the transportation burden for families…

2. Further research (collaboration)…

  • Academic Pediatric Association Complex Care SIG, AAP Section on Home Care, AAP Council on Children with Disabilities: many PICU’s have trouble discharging patients because of lack of home care nursing
  • Families have to “fill in” because even if they have home care nursing, when home care nurses “cancel” visits, there are no “replacements”
  • Insurance does not necessarily cover for the appropriate number of hours of home care nursing
  • Some researchers believe that reduced quantity/quality of home care nursing may result in more frequent ED visits and return hospitalizations
