0-5 Health Visiting Service

  • The Health Visiting service will work with ALL children age 0-5, offering the mandated

and routine checks, and more targeted work as well when needed.

  • When there are more complex health needs or a family needs more intensive support,

they may be best placed with Strengthening Families (see further slides)

What they will cover:

  • Mandated checks and additional ‘contacts’ at 3, 6, 18 months and 3 years
  • Routine screenings
  • Attachment and bonding with your baby
  • Infant Feeding including breastfeeding support
  • Child Development including toileting, weaning, behaviour, sleep,

home learning environment

  • Speech, language and communication early support
  • Early identification of Developmental delay and Special Educations Needs

and Disabilities (SEND)

  • Being ready for School
  • Healthy Lifestyles including healthy weight (portions size, diet etc),

physical activity and play, looking after teeth etc

  • Supporting parent/carers emotional wellbeing

How we will deliver the health visiting service:

  • In family’s homes
  • Through virtual clinics with a Health Visitor
  • In Libraries, Family Hubs, GP practices etc
  • Through existing groups like toddlers, stay and play
  • In places like Nursery’s or Schools

5-19 Public Health Nursing including National Child Measurement Programme (NCMP)

  • The 5-19 Public Health Nursing Service work directly and indirectly with the 5-19 age group, supporting schools and colleges to provide a whole school approach to positive health and wellbeing.
  • They will offer individualised and targeted support to children, young people and families, working with them around the physical and emotional health and wellbeing needs of their child.
  • They will support Early/Family Help Assessments where there is a health need identified (and no other health professional identified/engaged) including when it is preventing accessing education and non-clinical needs.

What they will cover:

  • Transition from Nursery/School/College where there is a health and wellbeing need
  • Healthy Weight advice and support for families and schools
  • Healthy Relationships including Sexual Health
  • Health and Wellbeing advice – sleep, behaviour, toileting/continence, emotional resilience, other health issues (non clinical)
  • Health Care Planning (not delivery) for children with additional health needs
  • ‘Healthy School/setting offer’ including Training for staff and parents/carer sessions
  • National Child Measurement Programme (NCMP)

How we will deliver 5-19 Public Health Nursing:

  • Virtual clinics (E-School Nurse) for parent’s/carers, professionals and young people where appropriate
  • Through Schools and other settings such as Family Hubs, Libraries etc
  • In family’s homes where appropriate

Strengthening Families

  • The Strengthening Families service will work with children, young people and families where there are more challenging and complex issues and health needs offer an increased number of visits and enhanced packages of care
  • Antenatal - work with all vulnerable families identified from 12 weeks of pregnancy onwards. This would include young parents, parents with SEND, Care experienced parents, where are more complex health needs including Early Help). They will work with these families until appropriate to transfer to the Health Visitor.
  • 0-5 years – work with families who are at Child Protection but only when there is complex health need which needs a more intensive offer of support (otherwise they remain with the Health Visitor).
  • 5-19 years - work with complex Early Help, Child in Need and Child Protection where there is a health need identified and no other health professional already working with the family.

What they will cover:

  • Families living with high numbers of Adverse Childhood experience (ACEs), mental health

and resilience issues, attachment disorders and presenting behaviour difficulties.

  • Liaising with and linking other services
  • Support where there has been trauma and offer advice and packages of support
  • Parental wellbeing
  • Enhanced attachment interventions
  • Listening visits to support maternal mental health
  • Enhanced monitoring of child developmental and progress
  • Enhanced support for emotional development, regulation and resilience
