WHO WE ARE AND WHAT WE DO

  • Complete PUNS enrollments and annual PUNS updates

  • Conduct Pre-Admission Screenings (PAS) of clinical eligibility and linkage to service providers 

  • Provide Individual Service and Support Advocacy (ISSA) to individuals enrolled in the home and community-based waivers (HCBS)

  • Develop and monitor the person-centered plan and address problems related to outcomes and service quality

PRIORITIZATION OF URGENCY OF NEED FOR SERVICES (PUNS)

  • Statewide waiting list for services for children and adults with I/DD who want or need Developmental Disability services but do not yet have funding
  • As funding becomes available, DHS/DDD selects individuals from PUNS from the Seeking Services category
  • Time does not accumulate while in the Planning for Services category
  • Selections are now made based on an individual’s length of time in the Seeking Services category
  • Selections will NOT be made from the Planning for Services category

PUNS – CATEGORIES OF NEED

  • Seeking Services – for individuals who currently need or want supports

  • Planning for Services - for individuals who do not currently want or need supports but may in the future

  • To move from one category to another, individuals/guardians must notify their ISC as this is not automatic

�PUNS - ENROLLMENT AND UPDATES

  • Intake call with individual/family

  • Individual Service Coordinator assigned to work with individual/family

  • Initial enrollments must be done face-to-face

  • PUNS are updated at least annually, or sooner when circumstances warrant

  • Annual updates can occur over the phone, in person or virtually

FUNDING CRITERIA

    • Community Crisis
      • Homelessness
      • Abuse (frequency, severity and duration of behaviors)
      • Neglect (what needs are not being met)
      • Loss of primary caregiver

    • PUNS Selection

�ELIGIBILITY FOR SERVICES

  • Intellectual Disability (IQ below 70), evidenced prior to age 22

  • Related condition such as Cerebral Palsy, Epilepsy, Autism Spectrum disorder or Traumatic Brain Injury that is found to be closely related to an Intellectual Disability

  • The condition results in impairment of general intellectual functioning or adaptive behavior like that of individuals with an Intellectual Disability

  • Requires services like those required for individuals with an Intellectual Disability and the condition manifested prior to the age of 22

ISSA�INDIVIDUAL SERVICE AND SUPPORT ADVOCACY

  • Provided to individuals receiving Waiver funded services by their local ISC agency

  • Provide support to the individual, their guardians, and providers in working through a variety of service issues

  • Designed to assist but not replace the guardian and other appropriate advocates

  • Writes the Personal Plan and determines whether Waiver funded services are being provided based on the Plan

  • Conducts quarterly monitoring visits

  • Ensure your son/daughter is enrolled on PUNS in the Seeking Services category
  • Apply for SSI
  • Apply for Medicaid
  • Contact the Division of Rehabilitation Services - Supported Employment or Home Services Programs
  • Contact your local Special Recreation Association - potential day program options
  • Save school records, assessments and evaluations
