I. ADMISSION�A. Requirements for Admission for Freshmen Enrollees�1. Form 138 (Report Card) duly signed by the authorities of the school last attended with Learner’s Reference Number (LRN)�2. Birth Certificate (LCR authenticated), NSO Copy if with annotations�3. Certificate of Good Moral Character�4. Certificate of Report of Rating (COR) of NAT (by August)�B. Transferees:�1. Certificate of Good Moral Character from the administrator of the school last attended.�2. Form 138 (Report Card) with Learner’s Reference Number (LRN)�3. Birth Certificate (authenticated), NSO Copy if with annotations� C. Old Students:�1. Form 138 (Report Card)�2. Students with failing grades and with record of disciplinary problems must be accompanied by the parents/guardians for interview by the year level head during the schedule of enrollment and for contract signing.
V. JURISPRUDENCE.� �All minor offenses or infractions of good discipline shall be handled first by class advisers. However, if the case is a grave offense, the class adviser is expected to refer the case to the School Discipline Committee. They shall call the attention of parents or guardians for a short conference (See Appendix A) and prepare the anecdotal report (See Appendix F). When the parties involved are between and/or among different year levels, the jurisdiction of school discipline officer shall be based on the year level of the primary offender. The School Grievances and Complaints Committee (SGCC) shall handle all sensitive cases and when the parties involved are between or among students and teaching and/or non-teaching staff.
�1. Students are required to attend classes regularly and punctually. Students are expected attend all scheduled school activities such as program, symposium, graduation, recognition and others.�2. Students are required to attend Flag Ceremony every Monday at 7:15 am and Flag Retreat every Friday at 4:15 pm.
VI. ATTENDANCE IN CLASSES AND SCHOOL ACTIVITIES
1. A student who has been absent shall be admitted to classes if he or she presents an Excuse Letter to the Teacher-Adviser/School Head stating the reasons for the absence duly signed by the parents/guardian.�2. Excused absences, like required absences in attendance to curricular activities in school, do not exempt the student from submitting required academic work such as reports, projects and others.�3. Students with (10) consecutive absences without valid reason is considered dropped after prior notice and approval of the parents/guardian.�4. If without reason and permission a student leaves his/her class after roll call has been made and does not come back is considered absent.�5. Irregular attendance of students must be justified by parents.
VIII. ABSENCE FROM CLASSES
1. A student is considered late if he/she is not in the line during Flag Ceremony (Monday). He/She is marked late if he/she is not in the classroom at the beginning of every period.�2. Five (5) tardiness is considered one (1) absent and is subject for disciplinary action.�3. Late comer must report immediately to their adviser for proper sanction.
IX. TARDINESS
1. Students may leave the classroom due to illness or for extreme necessity only with the permission of the teacher/nurse, adviser or School Head.�2. No students may be called from the classroom during class hours without authorization of the teacher or the School Head as the case maybe.�3. No students are allowed to leave the room and disturbed another class just to borrow materials, ask questions and others.
X. LEAVING THE CLASSROOM DURING CLASS HOURS
1. A student can leave the campus only with a written request duly noted by his/her adviser/School Head.

�A. During Flag Ceremony:�1. At the strike of 7:15 in the morning (Monday), students must form a straight line on their respective places facing the flag pole.�2. Students and teachers must stand straight quickly in their line while waiting for the start of the ceremony.�3. A morning prayer is the first part of the ceremony to be followed by the raising of the flag. Everybody participate in singing the National Anthem. Students and teachers are required to place their right palm on their breast while singing it. Students likewise teachers should raise their right hand while reciting the pledges. After the flag Ceremony, students must proceed silently to their respective rooms for the first period. (Item 3 is done in the classroom before the start of the first period class from Tuesday-Friday.)

B. In The Classroom:�1. Students are expected to observe and demonstrate conduct such as courtesy, politeness and respect to teachers, fellow students and to visitors. �2. During break time, free periods and class intervals, students may stay inside the classroom provided that they should observe silence. Students are not allowed to sit on the teacher’s table and chair.�3. If the teacher is not in the class in the prescribed time, students should wait quietly inside the classroom.�4. Cleanliness and orderliness in the classroom must be observed at all times.

C. During Assemblies/Programs:�1. Students must stay in their respective areas assigned to them by the teacher adviser. No one must leave the assembly area without the permission of the class adviser.�2. Students are expected to watch and listen attentively to the numbers or the program especially if it is about the theme of the celebration, information dissemination and message of the invited guests and the like.�3. Students should clap their hands gently in giving applause to the rendered numbers.

D. During Examination�1. Pens, pencil and other required materials for the test may be brought to the testing room.�2. No communication of any form is allowed between examinees.�3. A student who is caught in act or in attempt of cheating will receive zero (0) score with an equivalent of 0%.

E. Towards School Authorities and Others:�1. Courtesy is expected of all students at all times.�2. Signs of disrespect such as grumbling, stomping one’s foot, shouting and the like in the presence of the older persons, administrators, teachers and other school personnel are unbecoming of students and are subject to disciplinary actions.

Signs of disrespect such as grumbling, stomping one’s foot, shouting and the like in the presence of the older persons, administrators, teachers and other school personnel are unbecoming of students and are subject to disciplinary actions.

F. Regarding School Properties:�1. It is the duty of every student to protect the properties of the school and must not make vandalism.

G. School Uniform�1. Students are encouraged to wear the prescribed uniform every day from Monday through Friday except Wednesday which is the civilian day for the students.�2. The prescribed uniform for�Males: Black pants and white polo shirt with shoes and socks (colored undershirts are strictly prohibited.)�Female: Maroon skirt with white blouse, shoes and white socks 3. Students are required to wear their ID as part of the school uniform.�4. On civilian day, student should come to school in civilian dress on shoes/sandal and with ID. City Shorts and Sleeveless Blouses are not Allowed.

H. Haircut �The acceptable haircut for boys shall be at least 1 inch above the ear and at least 3 inches above the collar line.

14.1. Grave Offense.�The offenses punishable by suspension or expulsion depending on the seriousness of the offense stated in Paragraph 1.1.2, Section 1, Chapter III, Part IV of 2000 DECS Service Manual are considered grave offenses.

Threatening, coercing, agitating or intimidating any person such as defaming a fellow student, teacher, or any school authority or its agents or publishing and circulating false information about the school, its officials, its students or faculty members.

�14.2. Minor Offense.�The minor offenses or infractions of good discipline to school rules and regulations are the following:�1. Absenteeism, cutting of classes and tardiness�2. Wearing of earrings for the boys and more than a pair of earrings for the girls, outlandish hairstyles, painting of tattoo, over accessories and body piercing�3. Patronizing suspected prostitution den, gambling and pornographic places�4. Using profane language to insult another�5. Littering (plastic cups, bottles, candy wrappers or any waste) inside the school campus specially inside the classroom and corridors

�14.2. Minor Offense.�6.Loitering and staying inside or outside the school during class hour�7. Bringing, using and selling of pornographic materials�8. Public display of affection (PDA) like holding hands, hugging, kissing, necking, petting and fondling�9. Not giving letter to parents�10. Unruly behavior during assemblies, religious services, etc.�11. Going to restricted places�12. Selling stolen goods in school�13. Refusal to display school ID prominently and placing stickers and other objects on school ID�14. Sitting on tables, standing on benches as sitting with feet up and legs wide apart�15. Writing or drawing on a fellow student’s books and notebooks.

In case of irresponsible and undesirable behavior, the following sanctions may be imposed:� �Oral warning and written reprimand. The violator shall be reminded on his/her misbehavior and shall sign on the behavior contract (See Appendix D) together with his/her parent/guardian. The case should be recorded in the confidential log book.�
XV. PENAL PROVISIONS.
Probationary. The violator shall be required to undergo therapy. There are two types of therapy: the school therapy and home therapy. �In school therapy, the student shall be given school tasks that aim to develop his/her sense of responsibility, moral character and personal discipline under the supervision of guidance counselor, school discipline officer and class adviser. On the other hand, home therapy shall be given to student who needs parental guidance. He/she shall be given tasks that aim to develop his/her intra personal skills under the supervision of guidance counselor, school discipline officer, class adviser and parents/guardians.�
XV. PENAL PROVISIONS.
Suspension. The violator may be suspended for a fixed period. It will be recorded in the student’s anecdotal record (See Appendix G).� �Advise to Transfer. The violator shall be advised to transfer to other schools. ��Expulsion. The violator shall be banned on enrolment. �
