Case Report

Age 18, Female, College Student.

Patient Demographics:

Patient with Hypermobile Ehlers Danlos Syndrome, Mast Cell Activation Disease, Migraines, and POTS (Postural Orthostatic tachycardia Syndrome).

Diagnosis:

She reports being super tired during the day, and it is struggle for her to complete her classes and homework. Main symptoms are Chronic pain, brain fog, fatigue, headaches, gastrointestinal pain (bloating, constipation, and pain).

Patient Background

This patient is young but has a great support system in her mother.

She tries to make the best of her situation and wants to advocate for herself, but sometimes finds everything overwhelming.

Her relationship with food is difficult, as she realizes she needs to eat but food causing pain makes her not want to eat.

Her weight is normal for her height.

Nutrition Management for Chronic Pain

Able to help pinpoint offending foods and eliminate them.

Helped with GI symptoms a lot.

She ate lots of veggies and fruits, less sugary and processed foods, and enough macronutrients to meet her needs.

I had her use Kate Farms oral supplement to replace meals, as she would sometimes go the entire day without eating.

Her Symptom Survey was reduced from 128pts to 86 pts. This is a good start.

Difficulties with Diet Changes

Part of the reason her symptom scale did not reduce more is because it was a struggle for the patient to get rid of processed foods such as French fries and ranch dressing.

She did not want to feel left out when dining with friends.

She felt the difference in her pain level and fatigue level when she ate processed items, but it was still a struggle.

Other Factors Affecting Diet

Strong possibility of Gastroparesis

Gastroparesis is referred to as delayed gastric emptying. It is a disorder that slows or stops the movement of food from your stomach to your small intestine. Stomach muscles work poorly or not at all causing the stomach to take too long to empty its contents. Diabetes is the most common underlying cause because it can damage nerve cells, but in Ehlers Danlos Syndrome, we see a very high rate of it as well.

The patient was encouraged to get a GI motility study done.
