Oahu Transition & Diversion Program

Purpose: The Transition & Diversion Program is to facilitate the transition of individuals with significant disabilities from nursing homes and other institutions to home and community-based residences, with the requisite supports and services” and provide assistance to individuals with significant disabilities who are “at risk” of entering institutions so that the individuals may remain in the community.”

Definition of disability- a physical or mental impairment that substantially limits one or more major life activities of such individual.

Referral Process

  • Transition process is a collaboration between the consumer’s social network, facility staff members, and consumer.
  • Support team assistance on intake requirements such as signatures on intake form and Independent Living Plan
  • Understanding what is needed for smooth transition
  • Face sheet (preferably)
  • Type of disability
  • Power of attorney (If needed)
  • Preferably one week before discharge

Continued – Oahu Transition & Diversion Program

Develop Transition Plan in conjunction with Independent Living Plan (ILP)

Our staff can continue working with consumer post-transition to assist with issues that may arise.

Post Transition survey

Consumer can also choose to be included in other independent living skills trainings, workshops, and peer support groups as needed.

  • Basic clothing
  • Basic furniture
  • Personal assistance services
  • Home modification to make home more accessible
