  • Copy of 6th grade physical submitted to nurse as it is completed
  • Anticipation for 7th grade requirements

- Booster dosage of tetanus vaccination (Tdap)

  • Students are not permitted to self-administer medication in school (all meds - prescription and nonprescription - can only be administered by the school nurse)
  • Physicals are required annually for interscholastic sports – they are valid for 13 months

Special Education Parent Advisory Council

  • Who we are: A network of parents, guardians, educators and other community members who work for the understanding of, respect for and support of, all children with special needs in the community.
  • What we do:
    • Advise the District on matters pertaining to children with special needs;
    • Share information and discuss matters of common concern and interest;
    • Offer speaker events, workshops, and other programs for families and children
    • Fundraise to provide support to the schools, offer events and programs, award scholarships. etc.
  • Meetings: Sept 18th 7pm and most 3rd Tuesdays of the month at the Blake Middle School Library
