Hospital Acquired Infections

Cause infections including pneumonia, bloodstream infections,

wound or surgical site infections, and meningitis in healthcare settings. Gram-negative bacteria are resistant to multiple

drugs and are increasingly resistant to most available antibiotics. Gram-negative infections include those caused by

Klebsiella, Acinetobacter, Pseudomonas aeruginosa, and E. coli., as well as many other less common bacteria.

Hospital-Acquired Infections (HAIs), or nosocomial infections, are infections that patients acquire during their stay in healthcare facilities, particularly affecting neonates due to their underdeveloped immune systems. Premature infants are especially vulnerable, with infection rates increasing as birth weight decreases.

Common Types of HAIs in Neonates

  1. Neonatal SepsisBloodstream infection, often from IV lines or catheters
  2. Ventilator-Associated Pneumonia (VAP)Due to mechanical ventilation
  3. Necrotizing Enterocolitis (NEC) with Infection – Severe intestinal infection
  4. Urinary Tract Infections (UTIs) – Due to prolonged catheter use

Risk Factors for HAIs in Neonates

  • Prematurity & low birth weight
  • Prolonged hospital stay (especially NICU)
  • Use of invasive devices (IV lines, ventilators, catheters)
  • Use of broad-spectrum antibiotics (leading to antibiotic resistance)
  • Poor hand hygiene & infection control practices

Preventing Ventilator-Associated Pneumonia (VAP) in Neonates

Ventilator Associated Pneumonia is a serious hospital-acquired infection (HAI) that affects neonates who require mechanical ventilation in the Neonatal Intensive Care Unit (NICU). Prevention strategies focus on minimizing infection risks, optimizing respiratory care, and maintaining strict hygiene protocols and Infection Control Measures.

Prevention Strategies in Neonatology

  • Strict hand hygiene for healthcare workers and caregivers
  • Personal Protective Equipment (PPE) : Gloves, masks, and gowns are used when handling neonates, especially for those in isolation.
  • Minimal use of invasive devices (and early removal if possible)
  • Proper Sterilization & Disinfection of equipment and do not reuse disposable or single use items
  • Environmental Cleaning and Disinfection of high touch surfaces.
  • Encouraging Kangaroo Mother Care (KMC) for immune support
  • Early detection & prompt treatment of infections

Viral Infections:

  • Respiratory Syncytial Virus (RSV)
  • Cytomegalovirus (CMV) infections
  • Herpes Simplex Virus (HSV) in neonates
  • Rotavirus or Norovirus (causing diarrhea)
  • COVID-19 exposure or infection

Fungal or Parasitic Infections:

  • Candida infections (especially in preterm neonates on antibiotics)
  • Congenital infections (e.g., Toxoplasmosis, Syphilis, Rubella, Cytomegalovirus, Herpes – "TORCH" infections)

When to Isolate a Neonate?

Neonates should be isolated in the following conditions:

Bacterial Infections:

  • Sepsis (Bloodstream infection)
  • Methicillin-Resistant Staphylococcus aureus (MRSA)
  • Multidrug-Resistant Organisms (MDROs)
  • Necrotizing Enterocolitis (NEC) with sepsis risk
