Family Teletherapy for Elementary School Students
This 12-week teletherapy program is designed to treat kids ages 5-12 by working with their parent(s) to teach them how they can best support their child through these difficulties. Daybreak’s qualified mental health clinicians work with parents to provide customized guidance, support, and coaching to help with their child’s specific issues.

We are thrilled to share that we have partnered with Daybreak Health, a school-based mental health company, to provide teletherapy services to our students ages 5 and up, and their families, with qualified clinicians. These services will be provided at no cost to families throughout the year.
When kids are mentally healthy, they can think clearly, pay better attention, and learn new skills. These evidence-based teletherapy programs have proven to positively impact academic outcomes like behavioral improvements, better grades, and higher attendance.

Teletherapy Program for All Students
Our district has partnered with Daybreak Health, a school-based mental health company, to offer teletherapy services to our students, ages 5+, with qualified clinicians. These services will be provided at no cost to families.
The program looks different if it is an elementary-aged student or a middle or high school student participating in teletherapy. If it’s an elementary-aged student, the program is designed to include their parent/guardian in the sessions. If it is a child who is over the age of 10, they can meet one-on-one with their therapist.
These evidence-based programs are meant to help kids understand their emotions and learn new skills and strategies to manage their symptoms that they can use in class, at home, and throughout their life.

Ahora estamos ofreciendo servicios de teleterapia para nuestros estudiantes, sin costo para ti ni tu familia. Este es un programa de 12 semanas en el que te reunirás individualmente con un terapeuta elegido específicamente para ti y tus necesidades. I have submitted a referral for {Student Name} to participate in teletherapy with a clinician through Daybreak Health, our partner in providing student mental health services. These services are offered at no cost to families. Since the sessions are virtual, there is flexibility in when you and your child can meet with the clinician.

Please look for an email that will be sent to you from Daybreak Health which will provide a link to provide consent, sign a release of information form, and schedule a 50-minute virtual intake session if it has not already been scheduled.

My child has been referred for teletherapy services. Now what?
Please look for an email that will be sent to you from Daybreak Health which will provide a link to the parent onboarding packet. It’s important that you complete this information as soon as you receive it to ensure that your child is connected to care quickly.

What if I need help completing the forms?
If you don't receive an email from Daybreak or need support, contact Daybreak’s Care Coordination team at carecoordinator@daybreakhealth.com or 415-992-6155. Please note that you may receive a call or call back from a member of our Care Coordination team with an out-of-state phone number.
What happens after my child is matched with a clinician?
Once a match is made with a therapist, you and your child will be informed via email and given a weekly session time. You will meet with the clinician weekly, over Zoom, for 50 minutes for approximately 12 weeks. During that time, the clinician will provide customized guidance, support, and coaching tailored to your child’s specific needs. You as the parent/guardian will always attend the sessions also.
