How we got here

general principles

  •   Interested families complete an assessment and need support in 3 or more identified domains.
  • The Team Coordinator meets weekly 1:1 with the family to identify long and short-term goals.
  • All plans will be family-driven and based on what the family identifies is best.
  • Team Coordinator works with the family to put all the pieces in place for the resources the family needs and wants. The Team Coordinator identifies supportive team members based on family’s individual needs and invites them to form a team.
  • Team Coordinator provides monthly family team meeting facilitation; coordination before, during, and between meetings; and ongoing 1:1 weekly support to families.
  • Flexible funds of $1,000 per family will be available annually to address unmet needs.
  • Team Coordinator maintains a close relationship with Family Team and Pilot Planning group to solve problems, cut through red tape, and address any team member changes and/or needs.
