  • Document consultations and additional supports with more accountability.
  • The new form allows for specific services, aids, supports to be identified with minutes.
  • ESY and transportation are more clearly documented.
  • A new Transportation worksheet has been added.
  • A list of students with supplementary supports can be generated for providers.
  • A service log can now be completed to document supplementary services/supports.

Supplementary services, aids or supports

  • Previously listed on the LRE form consultations/additional supports section below the other placement rationale.
  • Services in this page are not necessarily connected to an IEP goal.
  • May be consultations/check-ins between educators, educator-student, or for equipment or adjustments.
  • The service, aid, or support is for student and/or staff/personnel.
  • Training for staff at the beginning of the school year or when there is a transition to a new site/classroom/staff.
  • Aids, Services, and/or Supports is an open menu, where you can type in more details.
  • If you are not sure about whether to write a certain service, aid or support on this form or the SIS cover page, contact your SPED director for guidance.

ESY

  • Use the ESY consideration checklist to assist the team in determining if there is a need. (Tools/Document Library)
  • If yes, enter the number of days, services, location, frequency, duration and provider.

Transportation

  • Use the Transportation worksheet to assist the team in determining if there is a need for the regular school year and ESY. (Tools/Document Library)
  • If yes, enter the level, emergency drop off and special requirements, if any.

  • The transportation worksheet can be found in the Document Library (English and Spanish) forms.
  • This may be used to assist teams in identifying student transportation eligibility and level of service needed.

  • SIS page has been updated to reflect the components of the new Supplementary/ESY/Special Transportation.
  • If any of the boxes are checked here, they will make the corresponding form required.

  • Go to Reporting/Student Lists/Edit List. Select Supplementary Services from the menu and Update List.
