Program Model

  • Partnership with West Side Campaign Against Hunger and NYP/CU & ACN
  • Target population: 300 NYP ACN patients experiencing food insecurity
  • Participants receive a 40lb healthy grocery box twice a month for 6 months plus connection to virtual social services
  • Referral model: Referred patients are either connected to the ANCHOR (WALLE+CRC) team for community food pantry referrals or to NYP CHALK for enrollment in Food FARMacia
