  • Community Centered Boards (CCBs) provide Case Management for Home and Community Based Services (HCBS) for individuals with Intellectual and Developmental Disabilities or Delays (under age 5)

    • We consider what is important to and important for people and develop individualized plans

      • Comprehensive/Developmental Disability waiver—18+ (waitlist)

The Intake Process:

  • Contact Envision’s Intake department for an application packet. Vicky Jacobson at vjacobson@envisionco.org 970-313-2606

  • An Intake Case Manager will be assigned to you once you submit an application. They will contact you to discuss the process, required documentation needed, and what the next steps are

  • You can apply for your child at any age, the sooner the better in order to plan for their future

Disability Determination:

  • Developmental Disability must be present prior to age 22
  • Medical diagnosis of a neurological or other condition resulting in cognitive impairment, for example: Autism, Cerebral Palsy, Down’s Syndrome, etc, and NOT caused by a mental health diagnosis

  • Adaptive Skills testing with a score of 70 or below, within 3 years

OR

  • IQ testing with a score of 70 or below, completed within 10 years (depending on the age of the person at the time of application)
  • Having an IEP or receiving SSI does not automatically mean a person has a developmental disability

Enrollment—Getting Services Started

  • You will meet with your Case Manager to discuss what services you are interested in receiving and to develop a Service Plan
  • Your Case Manager will send out a referral to all Program Approved Service Agencies (PASAs) for the services you want
  • PASAs will respond to the referral if they are able to provide the service and your Case Manager will forward you the response
  • You will contact the PASAs that responded to see if you would like to receive services from them. This will be added to your Service Plan

Case Manager Responsibilities

  • Your Case Manager is an expert in waiver services and will provide advocacy as well as answer any questions or concerns you may have
  • Your Case Manager will provide quarterly monitoring and change or update your services if needed
  • The typical timeline for starting services is 45-60 days from the Service Plan meeting
  • At least 1 service per month must be rendered in order to remain active on the waiver
