Surveyors:

  • DSCT.05-5 EMS record/prehospital care not captured in inpatient EMR
  • DSCT.05-4 ICH patient with change in NIHSS, RN note was good and had clear documentation who she notified, surveyor wanted to see a follow up note written by ICU APN on plan of care 
  • DSSE.01-1 TNK risks and benefits discussion documentation missing (this was the only patient it was missing on, patient presented early with posterior stroke symptoms and opted for MRI prior to TNK so tele-neurologist wasn't on camera at time) 

Other areas of focus:

  • CT tech and case management education- what stroke specific education do they get 
  • OB patient education on risk factors for stroke 
  • Discharge goal set by patient 
  • Driving after stroke 
  • Family readiness 
  • Pt exp project- liked our questionnaire but wanted to see a more clear PI project 
  • liked the Code ICH project 
  • Overall had a lot of positive things to say about the program

ISC Highlights

  • Opening Session, the trials found:
  • Minimally invasive surgery for intracerebral hemorrhage did not improve longer-term patient outcomes (MIND/ENRICH)
  • Transradial angiography offers advantages over transfemoral angiography.
  • In patients receiving best medical treatment for medium or distal vessel occlusion, adding endovascular treatment did not reduce disability and death, the DISTAL trial found.
  • Endovascular therapy provided no added benefit over usual care, results from the ESCAPE-MeVO trial showed for medium vessel occlusion.
  • The DISCOUNT trial found mechanical thrombectomy wasn’t superior to usual care for treatment of distal vessel occlusion.
