PERFORMANCE

Evaluation of the student's performance in the placement agency should be an ongoing process.

This means the student should receive continual assessment and feedback about their performance over the course of time at the placement.

MID-SEMESTER

EVALUATIONS

  • Conducted during the mid-point of the semester

  • A formal verbal conference (unless performance issues have been identified)

  • This evaluation is typically verbal unless the student’s progress has been determined as less than satisfactory.

DURING THE MID-SEMESTER EVALUATION THE STUDENT AND FIELD INSTRUCTOR SHOULD:

  • REVIEW OF THE LEARNING PLAN WITH REGARD TO PROGRESS;
  • DISCUSSION OF EACH OF THE STUDENT'S SIGNIFICANT ASSIGNMENTS TO DATE;
  • EVALUATE THE STUDENT/SUPERVISOR RELATIONSHIP WITH

REGARD TO TEACHING AND LEARNING STYLES, DIRECTNESS OF

FEEDBACK, AND/OR OTHER IDENTIFIED ISSUES

  • SUMMARIZE THE MOST SIGNIFICANT AREAS OF PROGRESS, AS

WELL AS AREAS OF MARGINAL OR UNSATISFACTORY

PROGRESS; AND

LIAISON

CONFERENCE

The field liaison will conduct at least one conference per semester with the student, field instructor, and task supervisor.

THE PURPOSE OF THE LIAISON CONFERENCE IS TO EVALUATE THE STUDENT’S:

  • SELF-AWARENESS
  • PROFESSIONAL BEHAVIORS
  • ABILITY TO ARTICULATE SOCIAL WORK PRACTICE AT AGENCY
  • CRITICAL THINKING
  • THE STUDENT’S LEARNING OPPORTUNITIES AT THE AGENCY

THE FINAL

EVALUATION

  • The final evaluation, provided by the School of Social Work, is required one week prior to the end of each semester of Field Instruction.
  • The final evaluation is an assessment of the student’s learning and demonstration of the core course competencies that must be completed by the field instructor.

The student must read the written evaluation and must sign it to indicate that they have read it, prior to giving it to the faculty field liaison.

 

If the student disagrees with any part of the evaluation, they may attach a signed addendum noting the areas of disagreement.

 

The field instructor must also sign the evaluation prior to giving it to the faculty field liaison for review and grading.

FIELD

TIMESHEET

In addition to the field evaluation, students are now required to complete and submit a log of their hours.

The Hours Log should be completed by the student, reviewed by the field instructor, signed during weekly supervision.

This document is due on the last day of field.
