THIS SECTION IS OPTIONAL

  • Use the “Outcome Type” drop down menu to select the kind of data you’d like to report.�
  • Enter your data into the “Outcome Data” box.�
  • Click the blue ‘Add’ icon to save your progress.�
  • Repeat as needed to include additional data. �
  • Use the pencil icon to edit or the X icon to delete information you’ve added. �
  • Click the ‘Next’ icon at bottom to move on to the next section.

Your additional outcomes data will populate at the bottom of the screen. You can view, edit, or delete them there.
