Where?

Setup>Program Setup>Policy/Consent

If a new form is being requested to be made a Category will need to be made first

Policy/Consent - Categories

Categories allows the user to create brand new forms and select where each can be seen.

  • Category: Title and name of the consent form
  • AZ and ZA:Filter options to list the forms Alphabetically from A-Z or Z-A
  • Reorder: to switch between alphabetical order
  • Order: the number associated to which order the forms with show

  • Fixed: This consent cannot be deleted
  • Show in Infograbber: The consent will show as an option to choose in infograbber
  • Show in TDO: The consent will be viewable in TDO

  • Show on website: When the patient logs into the office website, they will be able to view the consent form, however they cannot sign the form online
  • Required: The consent form is required for the patient to sign.
  • Consent per tooth/patient: If it is set per tooth, then the consent form can be assigned to a certain tooth on the case presentation page.
    • Common RCT Consent forms- if the patient is have multiple teeth done they will need to sign multiple consents per tooth.
  • Expire in Months: If you do not want the consent to expire, put 999.
    • Default options are Tooth will expire in 6 months and Patient will expire in 12 months.

  • Creating the text for a new form is best to be done in MS Word then copy and paste to the desired forms text box
    • Spell Check
    • Centering
    • Images
    • Printing

  • Red Boxes indicate that there is a missing signature.
  • To the left of the Signed On will show a box if the form is required to be signed

Changing the Text

If a Consent form is updated and saved it will mark any previous signed consent forms under that title as “Not Signed” since the form has been updated since the last signature was taken.

Case Presentation

If the office is done reviewing forms with the patient they can click to check consent forms as signed.

This message will only show if there are required or expired forms missing signatures.

Troubleshoot

Consent form was signed but not showing signed.

How can this be verified?

Infograbber could resend the signed documents showing which forms had been signed

Infograbber Signatures

  • After Continue is clicked the forms that were previously filled will then be shown
  • Any new additional information that may be needed can now also be added
  • Click next until signature is shown

Missing the Signature for Replicated Locations

  • Patient can not be found when looking in infograbber
    • Replica is likely down at the main location
    • Contact TDO for Assistance
  • Missing the signatures but is signed on the infograbber
    • Replica may also be down
    • Contact TDO for assistance
  • We will require the patients ID number for confirmation
