Household Data Collection Forms (HDC)

Why do we collect HDC Forms each year?

The Household Data Collection Form is completed annually by our parents. This confidential form may result in valuable funding for educational services at our schools.

*Every eligible form that is not submitted means lost funding for our schools. Your response helps the school qualify for programs and services that benefit students, and the information is kept confidential.
