Introduction

Welcome to the Google Forms Library.

These templates were specifically created for use in teaching.

Use them to collect feedback, help students reflect on key concepts, offer extension tokens, survey your class, and much more.

Instructions - setup

  1. Click the “Preview template” button under the form you would like to use (make sure you are signed into Google with your Ryerson ID)
  2. Click the “Use Template” link provided in the top right hand corner of the screen
  3. A copy of the template will open. Edit the form according to your preferences. Be sure to delete the “about this template” content in the description field. All your edits will be saved to your personal Google Drive.
  4. Under the “Settings” tab, you can adjust how student responses will be handled by the form.
  5. Click “Send.” Add the email addresses of your recipients, or click on the paperclip icon (circled) to copy a link to the form.

Instructions - responses

  • Once you have shared the form with students, visit the “Responses” tab
  • By clicking on “Link to Sheets” you can create a Google Sheet that is automatically linked to the form and will update as new responses come in. After it is set up, you can click on this link again to return to the linked sheet.
  • If you’d like to be notified whenever there is a new response, select the triple dots icon beside “Link to Sheets.” In the drop down menu, choose “Get email notifications for new responses”

Note: As submissions come in, the “Responses” tab will also include an overview of the results. However, you will likely find the Google Sheet (linked in Step 2 above) more useful for reviewing and analyzing the results in detail.

Extension Token

This form provides students with a token they can redeem for a short extension, no questions asked.

Some instructors may choose to offer only one token per semester. If you wish to offer two or more, simply make another copy of the template and rename it accordingly (e.g. “token two).

Note: Tokens are one element of specifications grading that can easily be implemented into most courses without the need to completely overhaul grading practices.

Assignment

Redo Token

This simple three-question form can be used for reflection or feedback. Note this version is not anonymous, but this can be changed in the forms settings if needed.
