  • Please report all medical issues/injuries to the athletic training staff. This enables us to work closely with you and our team physician. This, in turn, gets your athletes back on the field in a safe and timely manner.
  • Any athlete who sees a physician, for any reason, must submit paperwork from the physician detailing any restrictions, limitations, and other clearance concerns.
    • Lack of documentation may result in being withheld from team activity
  • Athletic trainers are bound by state law to follow physician’s orders

when an athlete has been seen by a physician.

Concussions

  • Brain Injury: All brain injuries are serious!
  • Possible symptoms: appears dazed, confused, or stunned; dizziness or balance problems, headache, sensitivity to light/noise, memory loss, unusually tired, nausea or vomiting, feeling slowed down, slow to respond, unusually sad or irritable, double or blurry vision, academic decline
  • If you notice any of these symptoms, see our athletic trainers or a doctor!
    • Any athlete with a suspected concussion, must be cleared by a physician and/or the athletic training staff, before being allowed back into any activity.
    • The athletic training staff follows a 5 day return to play progression for all confirmed concussions. They will explain in detail this progression through emails and phone calls to the parent.
  • You will have to complete a concussion acknowledgement form

Injury Documentation

  • The athletic training staff keeps medical records and documents injuries using an electronic medical record(EMR).
  • The specific EMR used by Hilliard Schools/OhioHealth is called Healthy Roster
  • Healthy Roster offers the ability for athletic trainers to communicate with parents via the Healthy Roster app.
    • Parents can get updates on their child’s injury status, see restrictions and limitations, as well as the overall progress of their injury from beginning to end
    • Parents using the Healthy Roster app can also upload physician notes and other pertinent information that needs to be relayed to the AT staff
      • Physicals still need to be uploaded to Final Forms only
  • The Healthy Roster app is optional for all parents
    • If a parent does not have healthy roster, an athletic training staff member will still contact the parent via phone or email about injuries

Injury Documentation

  • Invites for Healthy Roster will be sent out via email. This email will come from Healthy Roster
  • The invite will include a link to download the app as well as instructions on how to set up an account
  • Once the account is set up, it should automatically sync with your child’s medical record on Healthy Roster

Athlete Must Haves

  • Physicals are due prior to practices or tryouts. A current physical is required to participate.
  • Physicals are good for 13 months from the date of the doctor’s visit. Final Forms will have a print out to be taken to the doctor. Once filled out during the exam, upload two pages, Physical Exam Form and Medical Eligibility Form, to Final Forms.
    • Physical must be in Final Forms to be considered valid.

Lindsay’s Law

Symptoms/warning signs of possible Sudden Cardiac Arrest?

  • Chest pain/discomfort
  • Unexplained fainting/near fainting or dizziness
  • Unexplained tiredness, shortness of breath or difficulty breathing
  • Unusually fast or racing heart beats

What happens if an athlete experiences these symptoms?

  • If the AT staff or coach recognizes that an athlete is experiencing these symptoms, and they cannot be explained by previous history or environmental factors, the athlete MUST be removed from activity immediately.
  • The athlete must then be seen and cleared by a health care provider before returning to activity

Bradley High School Eligibility

  • 9th Grade: Must have earned a 1.5 in the preceding grade period
    • Probation: between 1.5 – 1.75 GPA
    • Waiver: May be taken if GPA is below a 1.5. Good once during 7th-9th grade years.
  • 10th -12th: Must have earned a 1.75 in the preceding grade period
    • Probation: between a 1.75 and 2.0 GPA
    • Waiver: May be taken if GPA is below a 1.75. Good once during their 10th -12th grade years.
  • A student must be enrolled and pass a minimum of (5) one credit courses or the equivalent. PE is a ¼ credit class and does not count.
  • Also – if enrolled in an online class or a class where college credit is being earned, please make sure how many credits it is worth at the high school level.

Memorial Athletics / Eligibility

THIS IS OUR 9th YEAR FOR THE 7-12 ATHLETIC

PROGRAM BEING ON THE SAME CAMPUS!

* We will have MS contests held in the Bradley venues!

Day of the Game

Attendance:

  • Students must be in school ALL DAY to be eligible to play that night.
    • If the game is on Saturday, they must be present on Friday
    • Excused items: College visits, family emergencies, religious celebrations, and doctors appointments for check-ups.
    • Unexcused items: Driver’s test, senior pictures, a day out with mom or dad, etc….

Transportation in Private Vehicles

Students may be permitted to drive or ride in private vehicles for trips under the following conditions:

1. No official school transportation is available.

2. They have pre-approved parental permission in writing to do so. Coaches will send this paperwork home to be signed.

3. They are legally allowed to drive (i.e., not operating without a valid license).

4. They are not in an obvious physical or mental state that suggests they should not.

5 Forms will be distributed by head coach for parents to transport their student-athletes.

Behavior and Discipline

Students may be removed from the team for the following offenses:

1. Use or possession of alcoholic beverages, drugs or other mind-altering substances.

2. The use of tobacco in any form (including Vape Pens).

3. Destroying or stealing athletic equipment or school property.

4. Violation of athletic manual rules, team rules, Board of Education policies, and/or OHSAA rules.

5. Any conduct unbecoming of a student-athlete (in or out of school, 24/7, 365) may lead to removal or loss of time in a sport. Including the inappropriate use of Social Media.

Uniforms and Equipment

  • Once the items have been issued to the athletes, it is their responsibility to take care of that property.
  • If anything gets lost or damaged, the athlete is financially responsible for those items.
  • Please have your athlete get a LOCK!

  • PLEASE CLEAN UNIFORMS AND EQUIPMENT PRIOR TO RETURNING THEM!

Coach Must Haves

  • According to OHSAA Bylaw 6: Coaches must meet the requirements of the Ohio Department of Education. They must have a Pupil Activity Permit (PAP) issued by ODE upon verification of the following:
    • Completion of Coaches Tool Chest
    • CPR
    • First aid
    • BCI/FBI Background Check
    • Mental Health Class
    • Completion the NFHS Fundamentals of Coaching course
    • Concussion
    • PAP
    • Sudden Cardiac Arrest Certification

Parents

Please know and demonstrate the Fundamentals of

Sportsmanship:

  • Respect, cooperate and respond enthusiastically to the cheerleaders, coaches and athletes of all teams. Censure fellow spectators whose behavior is unacceptable.
  • Be positive toward players and coaches regardless of the outcome.
  • Respect the judgment and the professionalism of the officials and coaches.
  • Please be aware of the Three (3) Offense levels for fans. You are subject to removal for inappropriate conduct.

  1. Athletes should approach the coach first.
  2. Parents should then contact the coach.
  3. Set up conference with AD and coach.

Realize that if you come to AD with concerns first, I will immediately go to the coach:

    • You may not hear what you want or expect to hear.
    • Anonymous letters and emails will be disposed of.
    • We do not know until we know.
