What is Different for Year Two-Personalization

  • Teams will do data analysis and determine a change idea as an Action Plan to meet their AIM goal
  • Teams will use the PDSA document to track progress.
  • The timeline is fluid and monthly measures are a priority. You have autonomy in the type and number of measures to monitor progress.
  • There is flexibility in the number of Action Plans/PDSA cycles you wish to pursue.
  • Coach will check in with the Improvement Team and review progress to include data measures.
  • Alignment with the LCAP Goals, Actions, and Services and your charter petition is your priority.
