Caudodorsal to the caudal endplate of L7 extending over the disc space a triangular mineralisation is visible (pink arrow), presumabely representing disc material of the slightly protruded inervertebral disc at the level of L7-S1. The disc protrusion causes no compression of the cauda equina, but at the level of the mineralized disc material, the left S1 nerve root is slightly deviated dorsally (blue arrow). The nerve root appears unremarkable.

The dorsal aspect of the kaudal L7 endplate is highly irregular with a heterogenous sclerotic pattern. The cranial endplate of S1 is minimally sclerotic and minimally undulating. Bone poliferations are visible in the ventrolateral aspects of the L7 body (turquoise arrowhead), causing minimal narrowing of the L7-S1 foramina. Bilaterally the L7 nerve roots have focal minimal contrast medium uptake (blue arrowheads).

lesion L7 endplate - an OC lesion is likely, due to the irregularity of the cortical bone a schmorl's node is unlikely, but an atypical form can not be ruled out. A discospondylitis is unlikely, due to the sole involvement of the L7 vertebra and lack of soft tissue involvement.
Dorsal displacement of the cauda equina - a tethered cord appears likely.
Presumed congenital sacral malformation
Disc protrusion L7-S1 with no cauda equina compression but slight displacement of the left S1 dorsally.
