What is a Google Form?

• A form is a survey linked to a spreadsheet
• Allows multiple users to input data that is dumped into a single spreadsheet for analysis

Getting started

Let’s ask your first real quiz question

A typical multiple choice question. Now make 2-3 of your own.

When this screen pops up...

Install and Run Flubaroo

Select "Flubaroo" and follow the directions to install it. It should show up under your Add-Ons drop-down menu after you've installed it.

Now grade your quiz

Tell Flubaroo what questions to grade and what questions to skip

Surveys

• Students can also survey each other for class projects
• Admins can survey teachers, parents
• Great for clubs, student government

Quizzes

• Either graded or ungraded
• Formative: project the summary data on screen to class, discuss Q's they had trouble with, reteach as necessary
• OR email each student their results w/questions and answers to help them learn from it on their own

Summative Assessments

• S's first record answers on paper
• S's then submit answers into either Version A or Version B form online
• Grade using Flubaroo
• No need for Scantrons! Save $$$!

Assignment Submission Forms

• Organize student work
• Sortable by last name, topic, class period, group number, etc
• Makes grading and presenting faster

Peer Feedback Responses

• Students give feedback to their peers during group/individual presentations
• You can later publish the spreadsheet with all the feedback to the whole class.
• Great for reflective writing
• You can import (as images) sections of your rubrics into your Peer Feedback forms (idea from @coach_sv)

Why Use Formative Assessments with Google Forms?

• Allows you to instantly find out what students know and don't know so you can re-teach to mastery
• Instant feedback to students (if you want) via email or presented to whole class
• Does not need to be graded
• Easily share data with your PLC

Ideas for Formative Assessments (@Roni_Habib)

• Diagnostic--before class, or before starting a new unit
• Dipstick--during a lesson
• Ticket out the Door--after a lesson--see if students understood what you taught them
• Jigsaw: Students create a lesson, teach it to the class, then assess students' understanding.

Summative Assessments

• You will keep making as many "Scantron"-style questions as you need. (Hint: You can copy the questions to save time)
• Make your answer key
• Be sure to title it "Version A" or "Version B" if you want to use multiple versions
• To make multiple versions, just copy the form and rename it.

Best Practices for Administering a Summative Assessment (@RoniHabib)

• Keep all those old paper tests! They are more secure!
• Make multiple versions like you would anyway.
• Make them put away all their tablets, smartphones, laptops (or collect them)
• Make the kids write down answers on a piece of paper.
• Collect the paper tests
• Give them back their device
• They enter the data into the Google Form
