We did not end up creating a specific template for a form for this purpose but I am including this photograph of one we were inspired by when we created this requirement. It becomes a place for the properties designer to gather all the information for the props in a scene by scene format that includes a picture of the item (This could be a photo of stock prop, personal item, photo of item for purchase, an inspiration photo with note that it would be built similarly.....) and it includes the ground plan (which the SET DESIGNER provides) so the props person can note where it would be used/stored...etc. DESIGNERS can create their own way of showing the information that works for them.

Important Elements to include:

Title of Play and Scene Number

Location (This example used a page per scene for props)

Groundplan Sketch for scene showing where prop is used in scene (This is something you would get from your SET DESIGNER in your group)

Visual reference with quantity needed. (See Examples on the following page)
