STUDENT FORM:
Petition type options will vary depending on the student’s program selected. The following options may display:
If Other is selected the following displays
CLA selection will ONLY display if a CLA program/plan is selected by the student.
Term selection option will vary depending on the Petition Type - Some options will only display current and past terms, some will display current and 1 future term, and some will show 1 future term along with current term and past terms
If Petition Type = Add a course(s) after the deadline
Depending on how many courses the student is adding (1-5) the following questions will be repeated for each of those courses.
When instructor’s internet ID is entered and “Lookup” clicked, the following will display.
The email field is editable if needed, if there is a suppression on the instructor’s account and email does not automatically display. Student’s should verify the internet ID is correct by asking the instructor, checking communications with instructor, or checking the university directory, before manually entering an email address.
If Petition Type = Change course level after the deadline

If Petition Type = Change grade basis of a class after the deadline
If Petition Type = Change variable credit value after the deadline

If Petition Type = Increase credit limit
If Petition Type = Program requirement substitution/modification
If the substitution/modification is not for the selected program, they have the ability to enter in the minor in order to help the Scholastic committee to forward the department approval.
If Petition Type = Swap a class section or course after the deadline

If Petition Type = Third attempt of a course
If Petition Type = Withdraw from a course(s) after the deadline (current or past term)
If student selects “withdraw from all classes for selected term” they will not need to complete any further information. If they want to withdraw from select classes, they must indicate the number of classes for withdrawal (1-5) then complete the following questions. These questions will be repeated for the number of courses indicated.
If Petition Type = Other
Students have the option to type in their personal statement or upload it as a separate document. Depending on option selected - one of the following will display:
Confirmation Screen: will display after submission.
College name and contact will display dependent upon program/plan selected.
