NYP Food FARMacia

Food FARMacia is a partnership between NYP CHALK and West Side Campaign Against Hunger to provide healthy groceries and virtual social services support to NYP/ACN patient households experiencing food insecurity.

Once enrolled, patients are scheduled for a Tuesday appointment to pick up a 40-pound grocery box (fresh produce, grains, protein, and dairy)

twice a month for 6 months.

Eligibility

  • Experiencing food insecurity
  • Has access to a kitchen
  • Able to pick up at 390 Fort Washington Ave, NY, 10033
    • Patients can send a friend, family member,

or aides to pick up for them if they are homebound

How to Refer an Eligible Patient:

  • CHALK and ANCHOR (CRC & WALLE) teams are working together to support patients experiencing food insecurity. Referred patients receive either social determinants of health screening and community resource recommendations or – for those already screened but experiencing continued need – enrollment in Food FARMacia

  • Providers (MD, NP, PA)
    • Refer using the referral order “AMB REFERRAL TO FOOD INSECURITY”
    • Dx Association – food insecurity

  • Other healthcare team members (social work, nutrition, patient navigators, CHW, RN, MA, etc.)
    • Refer via InBasket message to “P NYP FOOD INSECURITY POOL”
    • Use SmartText field box, search “food insecurity” to pull up

  • All referrers:
    • Please include notes where relevant
    • Specify one or both 2021 program options:
      • Emergency Food Assistance = community food pantry recommendations and/or Food FARMacia
      • Entitlement Enrollment = SNAP & WIC support
