HUD language examples of confusion

  • “A housing provider may not ordinarily inquire as to the nature and severity of an individual’s disability…However, in response to a request for a reasonable accommodation, a housing provider may request reliable disability-related information…”
  • “…In most cases, an individual's medical records or detailed information...”
  • A doctor or other medical professional… who is in a position to know about the individual's disability may also provide verification of a disability.

  • Once you have completed the steps above, please call…and set up a 30-minute Zoom appointment with a DAS Housing Adviser
  • Based on the information provided in your application, DAS may request the DAS Housing Documentation form filled out by a qualified healthcare provider
  • Once DAS receives the information above, we may request a 60-minute Zoom appointment with a DAS Housing Adviser
